Toy Cleaning Volunteer Application

Apply to earn volunteer hours by assisting with the toy cleaning at Helen Murphy Branch Library on Graham Street. Tasks will consist of using cleansing solution and paper towels to clean toys as well as documenting task completion. Toy Cleaning Volunteering is offered on Mondays and Wednesdays, from 4pm-5pm, meaning you may earn a maximum of 1 hour each time you participate. All spots to participate are first-come, first-served. A link to the Branch Teen Volunteer Calendar will be emailed to you upon completion of this application, which will allow you to register for future Toy Cleaning Volunteer shifts.

I,Parent / Legal Guardian's Name*, declare under penalty of perjury under the laws of the State of California that I am the parent or legal guardian of the minor applicant, Teen First Name Teen Last Name. I hereby give permission for my child / ward to participate in the Teen Volunteer Program at the Helen Murphy Branch of the Huntington Beach Public Library. I am aware that teen volunteers do many assignments independently and are not often supervised by staff. I do hereby indemnify and hold harmless the City of Huntington Beach, it's employees, volunteers, or agents from any liability for accidents, injuries, or illness that may occur to the applicant from their participation in the Teen Volunteer Program. In the event I cannot be reached in an emergency, I hereby give permission to the emergency medical staff selected to hospitalize and secure proper treatment for my child, and I will assume all medical costs. I hereby give consent to the City of Huntington Beach, its employees, and representatives, to use, publish and republish any and all photographs taken of my child . ward without further compensation. All photographs shall constitute the property of the City.
