By submitting my application and academic/official documents to Academic Credentials Evaluation Institute, Inc. (ACEI), I hereby declare and acknowledge the following:
1. I understand that the ACEI Application for Credential Evaluation serves as my contract for service and certify that I have read and agree to the TERMS & CONDITIONS outlined for the Credential Evaluation Service.I declare that all documents I am submitting are authentic, original, and unaltered records issued by the appropriate institution or authority.
2. I understand and agree that ACEI reserves the right to verify the authenticity of any document with the issuing institution or relevant authority at any time or request official documentation from the source institution at anytime.
3. I acknowledge that submission of altered, falsified, or fraudulent documents will result in Immediate cancellation of my evaluation;
Forfeiture of all fees paid; Notification to any institution, agency, or third-party recipient involved; Possible reporting to appropriate authorities.
4. I confirm that all information provided in my application is true, accurate, and complete to the best of my knowledge. By signing below, I affirm that I have read, understood, and agree to the above declarations and accept full responsibility for the documents I have submitted.
