I, PARENT/GUARDIAN OF THE YOUTH LISTED ABOVE, HEREBY CONSENT THAT THE PHOTOGRAPHS AND/OR MOTION PICTURES OF VIDEOTAPES WHICH SHE/HE POSED, AND/OR AUDIO RECORDING MADE OF HIS/HER VOICE MAY BE USED BY THE FORT MYERS POLICE ATHLETIC LEAGUE INC. (PAL), ITS ASSIGNEES, OR SUCCESSORS IN WHATEVER WAY THEY DESIRE, INCLUDING TELEVISION AND ELECTRONIC MEDIA. FURTHERMORE, I HEREBY CONSENT THAT SUCH PHOTOGRAPHS, FILMS, RECORDINGS, PLATES AND TAPES ARE THE PROPERTY OF PAL, AND THEY SHALL HAVE THE RIGHT TO DUPLICATE, REPRODUCE, AND MAKE OTHER USES OF SUCH PHOTOGRAPHS, FILMS, RECORDINGS, PLATES, AND TAPES AS THEY MAY DESIRE FREE AND CLEAR OF ANY CLAIM WHATSOEVER ON MY PART.

Participant-Parent/Guardian Zero Tolerance- Code of Conduct

All participants and parents/guardians will abide by the code of conduct which includes the following provisions. If any of thes rules are broken, the Fort Myers Police Athletic League,(PAL), shall have the authority to impose a penalty, remove anyone from any event and/or remove from participation in the PAL program.
- No smoking and.or use of smokeless tobacco at any PAL sanctioned event or practice.
- Will not critize players/spirit participants in front of spectators.
- Accept decisions of the game officials and judges on the field /court and in competitions as being fair and called to the best ability of said officials.
- Will not critize an opposing team, its players, spirit participants, coaches, or fans by word of mouth of by gesture.
- Strive to make every activity serve as a training ground for life and basis for good mental and physical health.
- Not engage in sideline coaching and shall not shout instructions from the sidelines.
- Together with team officials, be jointly responsible for the conduct and control of team fans and spectators. Any fan who becomes a nuisance and out control will be removed from the event.
- Not use abusive or profane language at any time or deliberately incite unsportsmanlike conduct.
- Not recommend or distribute any medication, controlled or over the counter, except as specifically prescribed by participant's physician.
- Not possess or drink alcoholic beverages and/or use illegal substance(s) at the game, practice, or PAL facility.
- Remove from a game or practice any participant when even slightly in doubt about his/her health, whether or not as a result of injury, until competent medical advice is available.
- Refrain from engaging in any action within or outside the event which reflects negatively upon or causes embarassment to the Fort Myers Police Athletic League Organization.
- Uphold all rules and regulations.
