Financial Responsibility

MediZen Revitalize operates as a self-pay practice unless otherwise specified in writing. Payment is required prior to consultation, prescription issuance, or medication processing.

I understand that medical approval and prescription issuance are based on provider discretion and medical eligibility. Payment does not guarantee approval of treatment.

Payment Authorization

I authorize MediZen Revitalize to charge my provided payment method for services rendered, including consultation fees, follow-up visits, and authorized treatment charges. I understand that recurring or future charges will only be applied for services I elect to receive.

Consultation & Service Fees

Consultation fees cover provider evaluation, review of medical history, clinical decision-making, and documentation. These fees are earned at the time services are rendered and are non-refundable once the consultation has occurred.

Medication & Pharmacy Charges

Medication costs are separate from consultation fees and are determined by the dispensing pharmacy. Once a prescription has been transmitted to a pharmacy and preparation has begun, medication orders cannot be canceled or refunded in accordance with federal and state pharmacy regulations.

MediZen Revitalize is not responsible for pharmacy processing times, shipping delays, or carrier-related issues once medication leaves the dispensing pharmacy.

Refund Policy

Due to the nature of telehealth services and custom-compounded medications, payments are final once services have been rendered or prescriptions have been processed. Duplicate payments or documented billing errors will be reviewed and corrected if applicable.

Chargeback & Payment Dispute Policy

I agree to contact MediZen Revitalize directly regarding any billing concerns prior to initiating a dispute with a financial institution. Initiating a chargeback for services rendered in accordance with this policy may result in termination of services and collection activity as permitted by law.

Cancellation & Rescheduling Policy

Appointments must be canceled or rescheduled at least 24 hours in advance. Failure to provide appropriate notice or failure to attend a scheduled appointment may result in forfeiture of the consultation fee.

MediZen Revitalize does not submit claims to insurance for weight loss or peptide therapies unless otherwise specified in writing. Patients are responsible for obtaining reimbursement directly from their insurance provider if applicable.

Returned or declined payments may be subject to additional processing fees.
