    PLEASE READ THE FOLLOWING CAREFULLY
  • In order to avoid a candidate having to pursue the process unnecessarily if a medical condition automatically results in refusal of the Transport Canada certificate, the CQFA would like to have some basic medical information to better guide the admission process.

Checking "yes" to the boxes does not mean that the applicant is systematically excluded from the process. If the condition suggests that the candidate is eligible for medical certification, the team will take the necessary steps with the candidate. If the condition represents an automatic exclusion, the CQFA will notify the candidate and explain the reasons.

NOTE: A false declaration will result in systematic exclusion from the selection process.
