Emergency Contacts and Authorized Pick-Up

Parents cannot be listed as emergency contacts. List the names of *at least 2 people* who can be contacted in the event of an emergency or illness if you cannot be reached. Any person listed should be able to assist in contacting you. Only people listed on this registration form will be allowed to pick up your child(ren) at the end of the camp day at your designated time. They must have identification or camper(s) will not be permitted to leave the camp grounds with said person.
Emergency Contacts and Authorized Pick Up Persons*

Select either "Full Week" or the individual days of the week for which you are registering your child(ren) for camp. If campers will be coming for different days/weeks, please complete a separate registration form for each child. *

Camp hours are from 8:30 a.m. to 4:00 p.m., Monday through Friday (except for holidays when camp is closed). Please complete the following if you camper requires after-hours care on the days they attend camp. Please note that additional fees will apply for campers who are not picked up by the end of after care hours.

A $50 non-refundable deposit must be paid via e-transfer at campjoy@wisdomandlifeapostoic.org to guarantee your child's registration. The balance of camp fees are due the day your child is scheduled to attend camp.

Terms and conditions
1. All registration forms must be accompanied by a $50 non-refundable deposit via e-transfer at campjoy@wisdomandlifeapostolic.org. Campers are considered registered when the deposit is received. The balance of your child's day camp registration fees are due, via e-transfer, by the first day your child is scheduled to attend camp. 

2. Although every effort is made to send campers home with all of their belongings, Camp Pure Joy is not responsible for any loss or damages. Please label all your camper’s personal items.

 3. Your child will follow all reasonable instructions from the staff of Camp Pure Joy while participating in the program. 

4. Camp Pure Joy reserves the right to cancel a child’s participation in a camp session if their behavior is deemed unmanageable or dangerous to themselves or others, in which case they will be sent home immediately.

5. You release Camp Pure Joy, its staff, volunteers and Wisdom and Life Apostolic Church of and from all manner of actions, claims and demands of whatever nature which result from any loss, injury, sickness, or expenses incurred, arising out of or in any way connected with participation in any program or attendance at a location operated by Camp Pure Joy. 

6. In the event your child is injured, ill, or in need of medical attention and you are unable to be contacted, you authorize Camp Pure Joy staff to seek medical attention on your behalf.

7. No refunds will be issued for instances of early dismissal for misbehavior.

8. All campers must bring their own sunscreen, wear closed toe shoes or sandals, bring a non-microwavable lunch and a JOYFUL attitude every day!
 
9. Persons authorized to pick-up campers must bring a photo identification card when picking up the camper. Campers will not be released to persons not included in their Emergency Contact and Authorized Pick-Up list.
