I hereby agree that I will not make a claim against Carole Devereux or Gualala Arts as a result of my or my animal’s participation in the workshop, and release Carole Devereux and the workshop facilitator, Gualala Arts, from all any and actions, claims, or demands I may have for injury resulting from my participation in this workshop.
I have carefully read and understand the contents of this agreement and am aware this is a contract for release of liability between myself, Carole Devereux, and the workshop facilitator, Gualala, Arts.
