  • Welcome to the EDGE 2026 Student Registration Form!

    We're so excited your student has been selected to attend the EDGE Youth Leadership Seminar — June 5th through June 7th, 2026 at Menlo College in Atherton, CA.

     
    Registration deadline: May 31st. All sections must be completed and submitted by a parent or guardian by this date for your student's spot to be confirmed.


    This form covers everything we need to ensure your student's safety and wellbeing during the weekend, including emergency contacts, medical information, transportation, and required consent and waiver agreements. It should take approximately 15–20 minutes to complete. You may save your progress and return at any time — you do not need to finish in one sitting.


    Before you begin, please have the following ready:

    • Your student's date of birth, personal email address, and phone number
    • Contact information for both parents/guardians if applicable
    • Your student's allergy and medication information, if any
    • Your preferred transportation plan for getting to and from Menlo College

  • Your Privacy Matters to Us

    EDGE Youth Leadership is committed to protecting the privacy of your student and your family. This notice explains how we collect, use, and protect the information you provide in this form.

    What we collect and why

    The information collected in this form — including student contact details, date of birth, medical and health information, dietary needs, emergency contacts, and demographic information — is used solely to register your student for the EDGE Youth Leadership Seminar, ensure their safety and wellbeing during the program, and fulfill our legal obligations as a youth-serving organization.

    Who has access

    Information is accessible only to designated EDGE staff and leadership on a need-to-know basis. We do not share your student's personal information with any third parties, including our host campus. Any information provided to partner venues or vendors is aggregate and non-identifiable — for example, the total number of students with dietary restrictions — and cannot be traced back to your student. We do not sell, rent, or otherwise share personal information for commercial purposes.

    How long we keep it

    Registration and consent records are retained for a minimum of three years following the seminar to meet nonprofit recordkeeping requirements. Health and safety information not required for ongoing records is deleted within 90 days after the seminar concludes.

    Your rights under California law

    As a California resident, you have the right to know what personal information we collect about you and your student, to request access to or deletion of that information, and to opt out of any uses beyond those described here.     Sensitive information

    Some information collected in this form — including health data, mental health disclosures, gender identity, and race/ethnicity — is classified as sensitive personal information under California law. For families offering carpool transportation, driver's license numbers are also collected and are subject to the same protections. This information is collected only for the purposes described above and is subject to additional protections. It will not be used for marketing, profiling, or any purpose unrelated to supporting your student's participation in EDGE.

  • Important Information About Medications at EDGE

    To help keep students safe during the EDGE weekend, we ask families to share information about medications their student may need while attending the seminar.

    EDGE Youth Leadership is not a medical or therapeutic program. However, EDGE staff will securely store and assist students with taking approved medications according to the instructions provided by the parent or guardian.

    All medications must be disclosed and provided at check-in in their original labeled containers.

  • Students occasionally need basic over-the-counter medications for minor issues such as headaches, stomach discomfort, allergies, or skin irritation.

    Examples may include:

    • Acetaminophen (Tylenol)
    • Ibuprofen (Advil / Motrin)
    • Antacids (Tums)
    • Antihistamines (such as Zyrtec or Claritin)

    In the event of an illness or injury requiring emergency medical care, and if I cannot be reached in a timely manner, I authorize the staff or volunteers of EDGE Youth Leadership to seek appropriate medical treatment for my child as deemed necessary by a licensed medical professional.

    I understand that EDGE Youth Leadership does not provide medical services and does not administer medications without parent or guardian consent, except as required in an emergency.

    I further understand and agree that I am financially responsible for any medical care provided to my child as a result of such emergency treatment.

    I hereby release, indemnify, and hold harmless EDGE Youth Leadership, including its officers, directors, volunteers, and agents, from liability arising from seeking or obtaining emergency medical care for my child, except in cases of gross negligence or willful misconduct.

  • EDGE Youth Leadership is not a clinical or therapeutic program. The question below helps our staff plan appropriate support and supervision during the weekend so we can respond thoughtfully if your student needs assistance.

    Information shared here is reviewed only by designated EDGE leadership on a need-to-know basis and will not be shared with other participants, partner organizations, or referring schools.

    A note on safety reporting: EDGE staff are trained mandated reporters under California law. If at any point during the seminar a student appears to be in immediate danger, staff are required by law to report to the appropriate authorities. This is a legal obligation and may occur without prior notice to parents or guardians.

    IN CONSIDERATION of the right to attend and participate in the aforementioned Activities, the Participant (and, if the Participant is a minor, their parent or legal guardian) hereby:

    1. Agrees to abide by all rules and regulations established by the EDGE Youth Leadership Seminar.

    2. Understands that every effort is made to provide participants with a safe, enjoyable, and memorable experience; attest and verify that the Participant is physically, mentally and emotionally capable of attending and participating in all activities offered at the EDGE Youth Leadership; acknowledges that there are inherent risks in any activity involving travel outside of one's own home or community, including, travel to and from the site, and, knowing the risks, nevertheless, agree to assume all risks of personal injury and any other losses and damage to person or property sustained while participating in, attending, and preparing for or traveling to and from the EDGE Youth Leadership; releases, waives and hold harmless the EDGE Youth Leadership, the sponsors, the volunteers, and officers, employees, agents, representatives, successors, and assigns from any and all liability or responsibility for injuries and/or property damage which Participant may sustain during the event or during travel to or from the event; understands that this waiver and release covers Participant (including all heirs, executors, or administrators) and is given in consideration of the EDGE Youth Leadership's acceptance of registration/entry into EDGE. This Consent and Acknowledgment of Risk shall not be amended, supplemented, or abrogated without the written consent of EDGE Board of Directors, San Leandro, CA.
       
    3. Agrees to defend and indemnify the EDGE Youth Leadership Seminar from any claim or action filed by a third party (against EDGE) due to their (the participant's) actions in this event.

    By signing below, the Parent/Guardian confirms that they have read, understand, and consent to the terms of this waiver agreement.

    EDGE Youth Leadership is a fully immersive experience where each session builds on the last. Full attendance is essential — students who miss portions of the program miss content that cannot be replicated.

    Students are expected to be present from Friday morning, June 5th through Sunday afternoon, June 7th.

    If a serious conflict would prevent full attendance, please contact us before May 31st to discuss whether an exception can be considered. Exceptions are rare and are reviewed on a case-by-case basis.

     

    Campus and Release Policy

    Once students arrive at the seminar on Friday morning, they are in EDGE's care for the duration of the weekend. This policy exists to keep every student safe and accounted for at all times.

    Students may not leave the seminar site for any reason unless one of the following applies:

    • They are being released to an authorized parent or guardian
    • They are accompanied by an EDGE staff member in accordance with our conduct policy
    • Students may not make independent transportation changes or arrange their own departure without prior approval from EDGE staff.


    If your student needs to leave temporarily for an urgent reason, this must be arranged in advance with our team. Students will only be released to the authorized parent(s) or guardian(s) listed in this registration. Please contact us as soon as possible if you anticipate this need — do not wait until the day of the event.

    Planned early departures or late arrivals are rare and reviewed on a case-by-case basis. Please contact us before May 31st if you foresee a conflict.


    School Absence on Friday

    Parents and guardians are responsible for ensuring their student's Friday absence is excused.

    • If your student was nominated by their school, the absence is typically excused — please confirm this directly with the nominating school contact.
    • If your student was nominated by a partner organization, please contact your school as soon as possible to request an excused absence.

    Please notify teachers and coaches in advance and arrange to reschedule anything scheduled for Friday, June 5th.

    Cancellation Policy

    If your student can no longer attend, please notify EDGE and the nominating school contact as soon as possible, and no later than May 31st. Most program expenses are committed and non-refundable by this date, and timely notice allows another student to be offered this opportunity.

    Cancellations after May 31st significantly impact the program. If cancellation is unavoidable after this date, we ask that families help the nominating contact identify a replacement student when possible.

  • The student's signature above represents a personal commitment to the EDGE program and is not a legally binding agreement. The parent/guardian confirmation above constitutes the legally binding consent.

  • Media Release

  • During the EDGE Youth Leadership Seminar, EDGE staff and volunteers may capture photographs, video recordings, and audio recordings of participants for use in materials that promote the mission of EDGE Youth Leadership. These materials may include printed publications, social media, our website, grant applications, and presentations to partner organizations and funders. Your student's image or recordings will never be used for commercial sale or advertising purposes.

    Declining this release will not affect your student's eligibility to attend EDGE.

  • Transportation

  • All transportation to and from the EDGE seminar is the responsibility of the student's family. Please select your student's transportation method below and complete the relevant details. If the student does not have transportation, EDGE may be able to help arrange carpool, but it is not guaranteed.

    Note for students driving themselves: Students who drive to the seminar will turn their car keys over to EDGE staff at check-in on Friday morning. Keys will be returned at check-out on Sunday afternoon.

    Important: Students under 18 may not travel to or from the seminar via rideshare or autonomous vehicle (including Waymo) unless a parent or guardian is present in the vehicle or at the pickup location. Waymo and similar autonomous vehicle services require riders to be 18 or older per their terms of service. Students may not independently arrange their own departure transportation.

  • EDGE reserves the right to verify driver eligibility and may request proof of insurance or license prior to the event. EDGE may decline a volunteer driver at its discretion.

  • Safe Transport Policy

  • To protect the safety of all students, EDGE Youth Leadership requires all volunteer drivers to follow these guidelines:

    • A volunteer driver may never be alone in a vehicle with a student who is not their own child. Every carpool carrying students from other families must include a minimum of two non-related students at all times, OR a second adult must be present in the vehicle.
    • Drop-off and pick-up must occur at the designated EDGE check-in and check-out locations at the event location. Students may not be dropped off or picked up at alternative locations without prior EDGE approval.
    • Volunteer drivers must not make unscheduled stops during transport of students.
    • Any change to transportation arrangements must be communicated to EDGE staff in advance.
    • Drivers must not transport students via ride-share vehicles (Uber, Lyft, etc.).
  • Carpool Waiver

  • I am voluntarily offering to transport student(s) enrolled in the EDGE Youth Leadership Seminar to and/or from the event. I understand that EDGE Youth Leadership is not a transportation provider and does not operate, supervise, or control private vehicle transportation. I assume full responsibility for the safe transport of any students in my vehicle and agree to comply with all applicable traffic laws and EDGE safe transport guidelines.

    I agree to release, indemnify, and hold harmless EDGE Youth Leadership, its officers, directors, staff, and volunteers from any and all claims, damages, injuries, or losses arising from transportation I provide to or from the seminar, including any accident, injury, or property damage occurring during transit.

    I understand that my participation as a carpool driver is voluntary and that EDGE makes no representations or warranties regarding the safety of carpool arrangements.

  • Closing Ceremonies

  • Parents/Guardians and family members are invited to attend the Friends and Family Program and Closing Ceremonies of the seminar. It is a fantastic chance to learn more what the seminar is about and see firsthand the excitement and energy created by the participants.

    Pick up a flyer at check-in for specific timing of programs.

    Check-out will begin after Closing Ceremonies, around 3:00 pm. As the time it takes to pack and check out varies, please do not plan on leaving the campus with your student before 3:00 pm. If you are not attending Closing Ceremonies and will not be present to help your student pack and check out, please plan to pick up your student after 3:45 pm, and no later than 5:00 pm.

    Students will only be released to individuals on the authorized pickup list below. Anyone picking up a student will be asked to show a valid photo ID at check-out. Students will not be released to individuals who are not on this list, regardless of their relationship to the student.
