By checking this box and registering for any Team Elite Academy program, I acknowledge that I have read, understand, and agree to the following terms:
Assumption of Risk & Release of Liability
I understand that participation in basketball activities involves inherent risks, including injury, illness, permanent disability, or death. I voluntarily assume these risks on behalf of myself and/or my child and release Team Elite Academy, its owners, directors, coaches, employees, volunteers, affiliates, sponsors, and facility partners from liability, claims, damages, losses, or expenses arising from participation, except as otherwise provided by applicable law.
Medical Authorization & Media Release
I authorize Team Elite Academy to obtain emergency medical treatment for my child if I cannot be reached and understand that I am responsible for any resulting medical expenses. I also grant Team Elite Academy permission to use my child’s name, image, photograph, video, and likeness for promotional, marketing, recruiting, website, and social media purposes without compensation.
Program Policies & Code of Conduct
I understand that all registration fees, tuition, camps, clinics, training sessions, memberships, tournaments, uniforms, and program payments are final and non-refundable. Team Elite Academy may modify schedules, practice locations, coaches, tournament schedules, and other program details as necessary. I also agree that my child and I will conduct ourselves with respect, integrity, sportsmanship, and professionalism at all times. Bullying, harassment, abusive language, poor sportsmanship, or disrespectful behavior toward coaches, teammates, officials, opponents, families, basketball organizations, or Team Elite Academy—whether in person or online—may result in suspension or removal from the program without refund.
Suspension or Removal
Team Elite Academy reserves the right to suspend or remove any athlete, parent, guardian, or spectator whose conduct is disruptive, disrespectful, or detrimental to the program, its participants, coaches, or families. No refunds will be issued if removal occurs due to misconduct.
I certify that I have carefully read and fully understand this Participant Waiver & Parent Agreement and voluntarily accept these terms on behalf of myself and/or my child.
