Membership Payment Authorization & Recurring Billing Agreement
By submitting this form, you agree to the following terms and authorize Albury's Advance Athletics LLC d/b/a The Grind Lab Tampa (“The Grind Lab Tampa,” “we,” or “us”) to charge your payment method as outlined below.
1. Recurring Billing Authorization
You authorize The Grind Lab Tampa to securely charge your selected membership fee to the payment method provided at enrollment on a recurring monthly basis, with billing occurring on the 1st day of each month.
You confirm that you are an authorized user of the payment method and approve recurring charges in accordance with this agreement until properly canceled.
2. Minimum Membership Commitment
All memberships require an initial three (3) month commitment beginning on the enrollment date. Membership fees during this initial term are non-refundable, except where required by applicable law.
3. Cancellation Policy
After completion of the initial three (3) month commitment, memberships may be canceled by submitting written notice at least fourteen (14) days prior to the next billing date.

Cancellation requests received fewer than fourteen (14) days before the billing date may result in one final billing charge.
Membership access continues through the paid billing period.
4. Failed Payments & Account Responsibility
If a payment is declined or returned:
The member agrees to provide an updated payment method promptly.
The Grind Lab Tampa reserves the right to reattempt payment.
Facility access or booking privileges may be suspended until the account is brought current.
5. No Refund Policy
Membership fees, training sessions, and services are non-refundable once billed or used, except as required by Florida law.
6. Chargeback & Payment Dispute Policy
By agreeing to these terms, you acknowledge that recurring billing authorization has been provided. Members agree to contact The Grind Lab Tampa directly to resolve billing concerns prior to initiating a chargeback or payment dispute with their financial institution.
Unauthorized chargebacks may result in suspension or termination of membership privileges.
7. Parent/Guardian Authorization (Youth Athletes)
If the participant is under eighteen (18) years of age, the parent or legal guardian completing this form:
Authorizes participation in training activities,
Accepts financial responsibility for all membership charges,
Agrees to all terms outlined in this agreement on behalf of the minor participant.
8. Payment Processing & Security
All payments are processed through a secure third-party payment processor. The Grind Lab Tampa does not store full payment card information.
9. Agreement Acknowledgment
By submitting this form, you acknowledge that you:
Have read and understand this Membership Payment Authorization & Recurring Billing Agreement,
Authorize recurring charges as described,
Agree to the cancellation and billing terms stated above.
