QMI Order Form: Student Data Questionnaires 2026-27

  • Questionnaire Management Interface (QMI) Order Form: 2026-27

  • Submit this form ONCE to request access to the Entering Student, Graduating Student, or/and Alumni Questionnaires (Qs) for the academic year 2026-27.

    ATS Student Data Services will distribute a full-color PDF report and raw data spreadsheet for each valid submitted batch (minimum 2 student responses). Combined reports will also be generated, if applicable. Refer to the QMI Administrator's Manual for more information.

    For past-year replacement reports, hard copies of current reports, or custom multiyear reports, contact Qmail@ats.edu as fees may apply.

  • Are you an employee of this school authorized to incur any fees related to QMI use?*

  • Please note that administrative access to the ATS Student Questionnaires may involve costs that will be invoiced to your member school. If you are not presently authorized to incur such fees, you must actively be seeking authorization from your institution. You may continue submitting this form, on the condition that you agree to notify ATS at Qmail@ats.edu to confirm your authorization. See pricing guide on the ATS Student Questionnaires webpage.

  • ATS recommends that your school designate at least one other QMI coordinator authorized to receive data results and reports as well as important Qs-related announcements.

  • Requesting Multiple Instances

    Having ATS create separate questionnaire instances to use with distinct audience groups at your institution may involve additional fees. If you are interested in setting up multiple instances, please reach out to ATS staff at Qmail@ats.edu to discuss your data disaggregation options for any extension sites, educational programs, or student cohorts (including specific graduating classes for the AQ). Alternatively, adding additional custom questions at no charge may suit your needs instead.

  • Those that express interest will recieve additional information and may be invited to opt in. Participation will be coordinated based on development priorities and available capacity. 

    Please note: raw data will be provided; however test reports are for evaluation purposes only and will not match current production reports. Functionality may vary, and timing and features may evolve. For questions, contact the@ats.edu.
