Coaching Agreement & Client Consent Form

Welcome! I’m so glad you’re here.

This form is your official agreement for participation in Foundations & Freedom Core (Group). It outlines the structure of the program, expectations for participation, payment terms, and community guidelines.

Please read through each section carefully and sign at the end to confirm your understanding and agreement.

This space is built on trust, clarity, and mutual respect and this agreement is here to support that foundation.

You are enrolling in:

Foundations & Freedom | Tier 4 | Core (Group)

This program is a 12-month coaching retainer combining group coaching and the Somatic Freedom Method™. The program begins on the date your first payment is received.

Your investment: $671 per month for twelve (12) consecutive months

Your enrollment includes:

• Two (2) live group coaching calls per month facilitated by Renee
• Access to the Somatic Freedom Method™
• Access to the private Skool community, where you can message Renee and connect with fellow members

Ready for deeper support? You can upgrade to a private coaching tier at any time to add 1:1 sessions with Renee. Contact us at support@reneejayne.com to learn more.

Continuation: 
As your 12-month program nears its close, we'll reach out about 15–30 days ahead to talk about your next chapter with Renee, whether that's continuing with a new 12-month program, moving to month-to-month coaching, or wrapping up. After we talk, we'll send a written confirmation of what we agreed to, and we'll ask you to let us know at support@reneejayne.com within 5 business days if anything doesn't match your understanding. Your billing will simply follow whatever we've confirmed together, and we won't continue billing you without that confirmation.

Practitioner Availability:
In the rare event that Renee is unable to continue coaching during your 12-month program due to illness, injury, or other unforeseen circumstances, we will work with you to either pause your program until she's able to resume, offer sessions with another qualified practitioner, or arrange a prorated resolution for the remaining term. Your commitment and payment schedule will not be forfeited without a resolution being offered.

Group calls are scheduled twice per month. If you are unable to attend a call:

• Calls will be recorded and shared inside the program platform.
• Recordings will be available for a limited time during the program term.

There are no make-up sessions for missed group calls.

This is a shared container.

Please:

• Show up fully present in a quiet, private space with camera on
• Avoid driving, multitasking, or distractions during live calls
• Maintain confidentiality of what is shared by other participants
• Refrain from recording, screenshotting, or redistributing content

Respectful participation is required. If community guidelines are violated, removal from the program may occur without refund.

Everything shared within the group container is confidential. You agree not to disclose personal information shared by other participants outside the program.

Renee will maintain confidentiality unless legally required to disclose.

This is a safe space for honest exploration. If something feels off for you, please raise it so we can address it together. This relationship is designed to support open and conscious communication.

All materials, recordings, curriculum, the Somatic Freedom Method™, and teachings are the intellectual property of Renee Jayne and Que Sera Media, LLC.

They may not be copied, shared, distributed, or taught without written permission.

All materials provided within this program are informational and educational only and are not medical or mental health advice.

Please consult your physician before making dietary or lifestyle changes, especially if pregnant, nursing, taking medication, or managing a medical condition.

Participation in any cleanse is voluntary and undertaken at your own discretion.

 

Coaching is a collaborative process that requires active participation.

Results vary based on many factors including personal commitment, effort, life circumstances, and individual implementation.

No guarantees are made regarding specific outcomes.

Your first payment is due prior to the start of the program.

By enrolling, you authorize automatic monthly payments via Stripe.

Payment plans are not subscriptions and cannot be canceled early. 

If a scheduled payment doesn't go through, we'll attempt to process it again and will reach out to update your payment method if needed. If a payment remains unresolved for more than 10 days, your sessions and access to program materials may be paused until payment is received. A pause does not change your remaining balance. You remain responsible for all payments for the full 12-month program, and unpaid amounts may be sent to collections if we're unable to resolve the issue together.

Because you receive immediate access to proprietary curriculum and materials, all payments are final and non-refundable.

By enrolling in this program, you agree to communicate directly with Renee Jayne Wellness at support@reneejayne.com to resolve any concerns prior to initiating a payment dispute or chargeback with your financial institution.

Submitting a chargeback after agreeing to this contract may be considered a breach of this agreement and may result in removal from the program.

This agreement is entered into by Que Sera Media, LLC (d/b/a Renee Jayne), and is governed by the laws of the State of Michigan, without regard to conflict of law principles. Any disputes arising from this agreement will be resolved in the state or federal courts located in Oakland County, Michigan.
