General Release and Waiver of Liability
In consideration of being permitted to participate in any program, event, activity, and/or volunteer opportunity organized by ourHOME, I hereby acknowledge and agree to the following:
- Assumption of Risk: I understand that participation may involve certain risks, including but not limited to physical activity, transportation, interaction with others, and/or use of community facilities. I voluntarily assume all risks associated with participation.
- Release of Liability: I, for myself and my heirs, executors, and assigns, hereby release and hold harmless ourHOME, its directors, officers, employees, volunteers, and partners from any and all liability, claims, or demands arising out of or related to participation in ourHOME activities, whether caused by negligence or otherwise, except in cases of gross negligence or misconduct.
- Medical Authorization: In the event of an emergency, I authorize ourHOME staff or volunteers to seek medical treatment for me (or my dependent) and understand that I am responsible for any resulting
- Compliance: I agree to follow all rules, instructions, and safety guidelines established by ourHOME staff and volunteers.
