The Monarchs Inner City Youth Program Registration

PLEASE READ CAREFULLY 

Thank you for your interest in playing for the Inner City Monarchs. Please realize this is NOT a recreational program and the focus on competition increases dramatically. The philosophy of this program is to give youth in the District 5 and 6 areas an opportunity to play baseball and softball at an advanced level while preparing them to play at the High School level.  

 

By Signing Up Below I/We understand:

- There is a significant time commitment to you team and teammates.

- This is NOT a recreational program and there is NO minimum playing time requirement.

- Most practices and games are mandatory.  Missing practices and games will affect playing time.

- Baseball and softball comes after school functions (ie. band, orchestra, science fair, etc.), major religious functions, and major family functions (vacations, family reunions, etc.).

- After April 4th baseball and softball comes before any other sport (ie. soccer, basketball, etc.)

- Coaches may hold some practices in the offseason (before April 4th). While these practices are not mandatory, every effort should be made to attend in order to avoid falling behind the other players (especially pitchers).

- I/We the parent(s) of a candidate for a position on a Inner City Monarchs baseball or softball team, hereby give my/our son or daughter approval to his/her participation in any/all activities during the upcoming season.

- I/We assume all risks and hazards incidental to such participation including transportation to and from activities, and I/We do herby waive, absolve, indemnify and agree to hold harmless the Monarchs organization, participants and persons transporting my/our child – except to the extent and in the amount covered by liability insurance.

- I/We understand Medical insurance will NOT be provided by the Monarchs .

- I/We will furnish a certified birth certificate upon request.

Payment of all fees are due at the yearly kickoff meeting.

INFORMED CONSENT & GENERAL RELEASE

Inner City Monarchs Baseball & Softball Program

PLEASE READ CAREFULLY. THIS DOCUMENT INCLUDES A RELEASE OF LIABILITY.

Participation in youth baseball and softball involves inherent risks. All participants and their parent(s) or legal guardian(s) must sign this Informed Consent and General Release prior to participation in any program activities.

RELEASE AND WAIVER OF LIABILITY

In consideration of my child being allowed to participate in the Inner City Monarchs Baseball and Softball Program, including all practices, games, camps, clinics, tournaments, team travel, and related events, I voluntarily release, waive, discharge, and relinquish any and all claims for personal injury, disability, death, or property damage that may arise from participation.

This release applies to injuries or damages that occur:

• During participation in any program activity
• At any facility used for practices, games, or events
• While traveling with the team (if applicable)
• While observing or participating in program activities 

This release includes claims resulting from negligence and is intended to release the Inner City Monarchs Baseball and Softball Program, its officers, directors, coaches, employees, volunteers, agents, and facility owners or operators from all liability to the fullest extent permitted by law.

ASSUMPTION OF RISK

I understand that baseball and softball activities involve risks, including serious injury, permanent disability, or death.

With full knowledge of these risks, I voluntarily allow my child to participate and assume all risks on behalf of myself and my child.

INDEMNIFICATION

I agree to indemnify and hold harmless the Inner City Monarchs Baseball and Softball Program, its officers, coaches, volunteers, agents, and facility owners, from any loss, liability, damage, cost, or expense resulting from my child’s participation in program activities.

UNIFORMS & EQUIPMENT

Each player will receive the following uniform items:

Two (2) jerseys
One (1) pair of pants
One (1) belt
One (1) pair of socks

Uniforms are issued for program use and must be properly maintained. Parents/guardians are responsible for ensuring uniforms are cared for and returned at the end of the season in good condition, reasonable wear and tear excepted. Failure to return issued items may result in replacement fees.

VIDEOGRAPHY & MEDIA CONSENT

I understand that practices, games, and program events may be photographed or recorded for training, promotional, and team-related purposes.

I grant permission to the Inner City Monarchs Baseball and Softball Program to use photographs, video recordings, or digital images of my child for:

• Team training and player development
• Program promotion and marketing
• Social media, website, and printed materials
• Highlight reels and recruiting purposes

I understand that no compensation will be provided for use of such media, and that images or recordings may be used without further notification.

If I do not wish for my child to be photographed or recorded, I understand it is my responsibility to notify the program in writing prior to participation.

FUNDRAISING COMMITMENT

I understand that fundraising efforts are an important part of supporting the Inner City Monarchs Baseball and Softball Program. I agree to support and participate in required team fundraising activities as communicated by the program.

If a minimum fundraising amount is established, I understand that I am responsible for meeting that requirement or covering the remaining balance as outlined by the program.

All fundraising efforts must be conducted respectfully and in accordance with program guidelines.

COMPLIANCE WITH PROGRAM RULES

I agree that my child and I will follow all rules, policies, and participation requirements established by the Inner City Monarchs Baseball and Softball Program.

If I become aware of any change in my child’s physical or mental condition that may affect their ability to participate safely, I will immediately notify a coach and remove my child from participation if necessary.

ACKNOWLEDGMENT

I have carefully read this Informed Consent and General Release.

I understand its terms and acknowledge that I am giving up substantial legal rights by signing it.

I sign this document freely and voluntarily.
