12X Decision Architecture

This inquiry form is designed for organizations, teams, and leaders seeking to explore a possible engagement with 12X Decision Architecture. Please answer as fully as you can; all information is confidential.
This intake determines alignment for a fixed-term structural engagement. It is not coaching or consulting. It is a documented operating intervention with written deliverables. Capacity is limited to two Blueprint engagements per month. All submissions are reviewed personally by Bruce Eickelman. If there is a fit, you will hear back within 48 hours. If there is not, you will be told that too. The form is not administrative. It is the first diagnostic instrument.
Section 1 — Operating Condition
Describe your current operating condition — not what you want it to be, what it actually is.
If you are unsure where to start, consider: What did you avoid dealing with this week? What decision has been sitting on your desk the longest? What conversation have you been putting off? You do not need to have the answer — describe the condition as accurately as you can.
What is the one decision you have been carrying the longest without resolving? How long has it been open?
Name it as specifically as you can. If there are several, name the one that costs the most to leave open. Include how long it has been unresolved — weeks, months, years.
What are you tolerating in the business that you know should not still be there?
This could be a person, a client, a commitment, a process, or a way of operating. Name the one that costs the most to keep tolerating.
What does a good week look like in your business right now — and how often does it actually happen?
Describe what a week looks like when the business is running as it should. Then estimate honestly: how many weeks in the last three months looked like that?
What have you already tried in order to resolve the condition you are describing?
List what you have already attempted — coaching, consultants, restructuring, reading, retreats, conversations. If the answer is nothing, say that. It is equally useful information.
Section 2 — Structural Signals
Which structural signals are currently present? Select all that apply.
If structural correction requires removing or redesigning commitments, are you prepared to act?
Is this decision solely yours to close?
Section 3 — Direction & Consequence
Where do you intend to be operating in five years — specifically how are you spending your time and attention?
Not revenue targets or growth metrics. How are you actually spending your days? What does the business look like without you in every decision?
If you stepped away for 30 days, what would fail first?
Be specific. Name the function, the person, or the process. This question reveals the structural dependency more clearly than any other.
What must change in the next 12 months for this engagement to matter?
Name the specific outcome. Not 'I want to feel less stressed.' What structural change, if made, would make this engagement worth the investment?
What decision, if closed, would immediately increase enterprise value?
Think about what a buyer or successor would see as the most significant unresolved structural issue if they were evaluating the business today.
Section 4 — Practical Context
Have you previously worked with a business coach, consultant, or advisor?
If yes — what was missing from that engagement?
You do not need to name the person or firm. What was the gap? What did you leave without that you needed?
Are you prepared to allocate 1–3 hours per week for 30–90 days to implement structural correction?
