Whakapākari Registration Form

Our Whakapakari Programme runs once a week on a Friday during the 2026 school term, beginning on 6 March. The programme is designed for young people aged 13–17.

Sessions are held at the PACT Building, 311 Great South Road, Papatoetoe.

The programme runs from 3:15am to 5:00pm.

Parent / Caregiver Consent:

All reasonable care will be taken to provide a safe and well-supervised environment for young people attending the programme, in accordance with PACT’s policies and procedures.
I / we will notify PACT promptly of any changes to enrolment or medical information.
Programme management reserves the right to exclude from the programme any young person who is repeatedly disruptive, does not follow programme expectations, or poses a significant risk to their own safety or the safety of others. The programme is committed to being fair, positive, and consistent in supporting appropriate behaviour.
If a PACT staff member witnesses or suspects neglect or abuse of any young person, they are required to follow safeguarding procedures, including notifying the appropriate authorities.

When you register a young person for Whakapākari you are agreeing to the above and by selecting the box below you confirm that you have read, understood, and agree to the following:

I understand that Papatoetoe Adolescent Christian Trust (PACT) is a Christian organisation. I acknowledge that PACT programmes may include karakia (prayers), discussions of faith, and conversations about hauora (overall well-being) that are grounded in Christian beliefs. I understand that participation in faith-based elements is invitational and that all rangatahi are treated with respect, regardless of personal beliefs.

I give permission for photographs and/or video footage that may include my child to be used by Papatoetoe Adolescent Christian Trust (PACT) for reporting, promotional, and marketing purposes. This may include use in print materials, on the PACT website, social media platforms (such as Facebook, Instagram, and YouTube), and in communications with funders and community partners. I understand that: My child’s name will not be published without additional permission. Images will be used respectfully and in a manner consistent with PACT’s values. I may withdraw this consent at any time by notifying PACT in writing.
