You must upload your Form 1040 (first 2 pages) AND all documents that show how your income was earned (W-2, 1099, or Profit & Loss Statement).
REQUIRED DOCUMENTS
• Form 1040 (First 2 Pages Only) – This is your income tax return summary.
• W-2 Forms – Provided by your employer (if you are employed).
• 1099 Forms – Provided if you are an independent contractor.
• Profit & Loss Statement – Required if you are self-employed (prepared by you or your accountant).
IMPORTANT CLARIFICATION
The Form 1040, W-2, and 1099 are separate documents. You must upload all applicable documents. Submitting only one document may delay your application.
