We maintain zero tolerance for abuse, neglect, exploitation, and reportable conduct. We uphold Participant rights, dignity of risk, and least restrictive practice principles. We comply with mandatory reporting obligations and the Reportable Conduct Scheme.

Wonder Kidz staff reserve the right, after all reasonable attempts to contact the Parent/Guardian or any Authorised Nominee listed on the enrolment form have been made, to seek medical treatment for the Participant from a registered medical practitioner, hospital, or ambulance service if deemed necessary. This may include transportation of the Participant by an ambulance service.* This field is required.

I consent to Wonder Kidz staff applying SPF50+ sunscreen to my child in accordance with the Wonder Kidz SunSmart Policy. I also agree to provide my child’s own sunscreen if required due to skin sensitivity or other personal considerations.* This field is required.

I acknowledge and understand the Incident Management System and mandatory reporting obligations for Reportable Incidents.* This field is required.Wonder Kidz Incident Management System ensures timely and effective management of all incidents, including Reportable Incidents as defined by the NDIS Commission. Participants and their Nominees will be informed and supported throughout the process.

I acknowledge the Child Safe Reporting and Complaints procedures and understand my rights.* This field is required.Parents, guardians, and Participants can make complaints internally via email and report concerns to the NDIS Quality and Safeguards Commission. Mandatory reporting obligations apply.

This service agreement is between the participant mentioned above and Wonder Kidz. These are referred to as parties in this agreement. This Service Agreement is made to provide support under the Participant’s National Disability Insurance Scheme (NDIS) Plan. All parties mentioned above agree that this Service Agreement is made in the context of the NDIS, a scheme that aims to: • support independence. Social and economic participation of people with disabilities in the planning and delivery of their support. This Service Agreement is for 2 years unless NDIS regulations state otherwise.* This field is required.Including NDIS auditors
