For better planning of our program's enrollment, we would like to be aware of any future plans you may have regarding your child's educational journey. Please complete this form if you plan on withdrawing your child from our program this Fall, to enroll in pre-kindergraten or kindegarten program. Even if your child has not yet registered, we ask that you complete this form. Please complete and submit this form by Thursday, April 30th.

NOTE: Please be advised that children who are currently 5 years of age would have met the age limit for our preschool program, no longer qualifying for our program. 

I acknowledge that I have shared with Little Rivers Christian Learning Center my child’s future educational plans and have informed them of my enrollment decision. I understand that if my child is transitioning out of the program, that this form will serve as a termination notice, effective the date stated above. I have read the policies regarding termination of services as stated in the Parent Handbook, and understand that my initial deposit can be credited for the last week of care, if no past balances or fees are due at the time.
