This form is for the 2026 Cockburn Esports Championship COMMUNITY day held in partnership with the City of Cockburn and the Australian Esports League.

This event is to be held 4th April 2026 at the Cockburn Youth Centre, 25 Wentworth Parade, Success WA 6164.

The event will run from 9:30am - 3:30pm, competitions begin 10:00am and finish 4:00pm.

This form is to be filled out per entrant by a parent or gaurdian on their behalf.

Spectators and anyone wanting to come along for some fun free-play matches may attend for free without filling out this form.

NOTE:

• Entry is FREE.
• Participants must be in years 6-12.
• Each competition is capped, any spare places closer to the competition date will be offered to existing participating schools.
• Participation is based on a first come first served basis.
• The event will feature some photography to capture the day.

Mixed Division (Years 6-12)

• FC (32 player cap)
