As the parent or legal guardian of the child named below, I herby give my full consent and approve for my child to participate in Worcester Little League sponsored activities.

I understand that there are certain risks of injury inherent in the practice and play of these club sponsored activities, as well as in traveling and other related activities incidental to my child's participation, and I am willing to assume these risks on behalf of my child. I herby certify that my child is fully capable of participating in the sport or activity sponsored and that my child is healthy and has no physical or mental disabilities or infirmities that would restrict full participation in these activities, except as listed below.

I hereby grant permission to the Worcester Little League and its representative, to take and use: photographs and/or digital images of my child for use in news releases and/or promotional materials. These materials might include printed or electronic publications, web sites or other electronic communications. I further agree that my child's name and identity may be revealed in descriptive text or commentary in connection with the image(s I authorize the use of these images without compensation to me. All negatives, prints, digital reproductions shall be the property of the Worcester Little League and its representatives.
