PARTICIPATION RELEASE:
By registering my child, I, their parent/guardian, do hereby agree to the following terms:
-I acknowledge that Day Camp 2025 at Northside Christian Church involves activities that carry inherent risks, including, but not limited to, outdoor play, group games, and physical activities. I understand that despite all precautions, accidents can occur.
-I hereby release, indemnify, and hold harmless Northside Christian Church, its staff, volunteers, and any affiliates from any and all claims, injuries, damages, or losses that may arise during my child’s participation in the Day Camp activities, whether or not caused by negligence or accident.
-In the event of an emergency, I authorize the staff of Northside Christian Church to seek medical treatment for my child if necessary and agree to assume responsibility for any costs associated with such care. I understand that every effort will be made to contact me or my emergency contact before any medical treatment is provided.
