Release of Liability & Assumption of Risk

I, the undersigned parent/legal guardian, understand that participation in activities with NTX Sharks Sports Academy involves inherent risks, including but not limited to serious injury or death. I voluntarily allow my child to participate in all practices, games, training sessions, travel, and related events and assume all associated risks. I hereby release, waive, and hold harmless NTX Sharks Sports Academy, its directors, coaches, volunteers, sponsors, and affiliates from any and all claims, liabilities, damages, or expenses arising from my child’s participation, except in cases of gross negligence or willful misconduct. I certify that my child is physically able to participate and that I will provide current medical information and insurance coverage.
Medical Authorization

In the event of an emergency, I authorize NTX Sharks Sports Academy and its representatives to obtain necessary medical treatment for my child. I understand that I am responsible for any medical expenses incurred.
Sportsmanship Requirement

All NTX Sharks athletes and parents must show respect, positivity, and good sportsmanship at all times. Unsportsmanlike behavior may result in disciplinary action, including removal from the program.
No Refunds

All payments are non-refundable. By registering, you commit to the full season and all associated costs.
Photo & Media Release

I grant permission for NTX Sharks Sports Academy to use my child’s name, image, and likeness in photographs, videos, live streams, and promotional materials (including social media, website content, and marketing materials) for program-related purposes without compensation.
