KDO Academy Elementary Application (K–6th Grade)

Application Fee & Payment Information:
- $80 per student
- $20 late fee for applications received after August 21, 2026

Note: Fee for bounced/returned checks: $25
Important Fee Policies:
- The application fee(s) must be paid to fully register. Application fee(s) are non-refundable unless your student is not placed in any requested classes.
- What the Application Fee Covers:
-Jupiter Online: $15 per student
-Cleaning Fee: $45 per student
-Processing Fee: $20 per student

The application is not complete until:
- Payment is received
- All required forms listed below are received

- Elementary Application Form
- Elementary Medical Form
- Elementary Safety Form
Tuition Rates (K–6th Grade):
- $30/week for the first child
- $25/week for each additional child (per family)
