Registrations for music classes are non-refundable and non-transferable. Registration prices cannot be adjusted for discount codes retroactively. The classes are scheduled for the hours listed on June 2, 9, 16, and 23. And, on July 7th ALL students (except Music Exploration) must be prepared and able to participate in a dress rehearsal 3pm-5pm. The showcase performance will be later that evening on July 7th at 6pm.*

I understand that my family will receive 2 reserved tickets for the showcase event on July 7th for every child registered for a class (except Music Exploration). For example, if there are 3 children in 1 family registered, that family will receive 6 total reserved tickets. Reserved tickets are required for entry of audience members (they are not needed for students) to ensure we have the right seating and a fair process for all.*

I take full responsibility (financially and otherwise) for the medical needs that may arise as a result of my family's participation at The Clubhouse in Holly Hill for music classes, including but not limited to allergies, minor first aid issues, accidents, communicable diseases etc. I understand that this program will be at a facility with a pool and agree to ensure my family reads and follows all posted signage for my safety.*

CONTRACTUAL STATEMENT for NON-MEMBER:

I understand the Clubhouse is a private country club, of which I am not a member, and therefore acceptance of this application for the program selected will be as a provisional guest of the facility. I recognize that, should I choose to pursue membership with The Clubhouse, my acceptance to this program is not a gaurantee of my membership application's acceptance and that any future membership status will not qualify me in any way for retroactive benefits associated with this program.

I hereby waive, release, and discharge The Clubhouse in Holly Hill, its staff, employees, volunteers, directors and/or coaches, any and all claims, liabilities, or causes of action that may arise from injury, accident, loss, or damage occurring during or resulting from my child's participation in any activity associated with this program. I give permission for my listed child(ren) to participate in all program activities. I affirm my child is physically fit and mentally capable of participating in all planned program activities.
