Thank you for supporting Project Sweet Peas through a third-party fundraising event. We are grateful for your willingness to help raise awareness and funds to support families navigating the NICU journey.

This form allows Project Sweet Peas to review and approve fundraising activities organized by individuals, groups, or organizations that wish to support our mission.

A third-party fundraiser is an event or activity organized and run by an individual or organization independently from Project Sweet Peas, with proceeds donated to support our mission.

Submission of this form does not guarantee approval. Project Sweet Peas will review the request and confirm approval before the event may be promoted.

Fundraising Commitment & Financial Guidelines
This fundraising event will benefit Project Sweet Peas.I understand that Project Sweet Peas must approve the event or activity before it is publicly promoted.I will clearly communicate how proceeds will benefit Project Sweet Peas.I will not open a bank account or collect funds using the name or tax ID of Project Sweet Peas.Net proceeds (gross revenue minus event expenses) will be submitted to Project Sweet Peas within 14 days of the event.

Event Responsibilities & Liability
Project Sweet Peas may provide guidance when possible but is not responsible for organizing, staffing, funding, or managing the event.

The event organizer is responsible for:
- Planning and executing the event or activity.
- Obtaining any required permits, licenses, or insurance.
- Complying with all applicable federal, state, and local laws related to fundraising or charitable solicitation.
Use of Name and Logo
Any use of the Project Sweet Peas name, logo, or branding must be approved in advance.

Approval of this does not grant exclusive rights to host fundraising events or activities benefitting Project Sweet Peas.

Cancellation
Either the organizer or Project Sweet Peas may cancel participation in the event at any time. Any funds already collected for the benefit of Project Sweet Peas must still be submitted according to the guidelines in this agreement.

Liability
The event organizer agrees to release, indemnify, and hold harmless Project Sweet Peas, including its officers, directors, employees, volunteers, and affiliates from any claims, damages, liabilities, or expenses arising from the organization or execution of the fundraising event or activities.
