Please read this form carefully and be aware that in registering yourself or your minor child/ward for participation in Summit Park District programs, activities, events, or facility use, you will be waiving and releasing all claims for injuries you or your child/ward might sustain arising out of these programs.
I recognize and acknowledge that there are certain risks of physical injury to participants in recreational programs, activities, camps, sports, and special events, and I voluntarily agree to assume the full risk of any and all injuries, damages, or loss, regardless of severity, that my child/ward or I may sustain as a result of participating in any and all activities connected with or associated with such programs.
I agree to waive and relinquish all claims I or my child/ward may have against the Summit Park District, including its officers, agents, servants, volunteers, and employees.
I do hereby fully release and discharge the Summit Park District, its officers, agents, servants, volunteers, and employees from any and all claims for injuries, damages, or loss that I or my child/ward may have or which may accrue to me or my child/ward arising out of, connected with, or in any way associated with these activities.
I further agree to indemnify and hold harmless and defend the Summit Park District, its officers, agents, servants, volunteers, and employees from any and all claims resulting from injuries, damages, and losses sustained by me or my child/ward and arising out of, connected with, or in any way associated with the activities of the program.
In the event of an emergency, I authorize the Summit Park District staff to secure medical treatment for myself or my child/ward if I cannot be reached. I understand that I will be responsible for any medical costs incurred.
I also grant permission for the Summit Park District to use photographs, video, or other likenesses of myself or my child/ward for promotional purposes including publications, social media, website, or other promotional materials.
By completing registration and selecting the agreement box, I acknowledge that I have read and fully understand the Waiver and Release of All Claims.
