2. Authorization to Register Participants - The "Registering Pary" represents and warrants that they have received permission from each participant and/or the participant’s parent or legal guardian to complete the registration for the TRIBE 99 event. The Registering Party agrees to provide all participants and their parents or legal guardians with the TRIBE 99 Participation Waiver & Release of Liability and any applicable Event Terms & Conditions. TRIBE 99 reserves the right to require individual waiver acknowledgement from participants or their parents/legal guardians prior to participation.3. Individual Waiver Requirement - All participants must have a completed Participation Waiver & Release of Liability signed by the participant or their parent/ legal guardian if the participant is under 18 prior to participating in any FLEUR event activities. Submission of a group registration does not replace the requirement for individual waivers. FLEUR reserves the right to deny participation to any individual whose waiver has not been properly completed. A link to the waiver will be emailed to the "Registering Party" and it is their responsibility to ensure it is passed along accordingly.10. Acknowledgement - By completing registration and signing below, I certify that (1) I have read and understand these Terms & Conditions. (2) I agree to all policies stated above. (3) I am the legal studio owner/director have received consent from parent/legal guardians to sign on behalf of any and all of the registered participants if under 18. (The Participation Waiver & Release of Liability also applies).

Participant Waiver & Release of Liability

1. Certification of Physical Fitness to Participate - I understand and have informed all registered participants as well as parents/legal guardians that participation in any sport, including dancing at FLEUR events, carries inherent risks of injury, including but not limited to serious permanent injury, paralysis, and death. I voluntarily and knowingly assume all such risks (if over 18) and/or on behalf of my registered students (if participants are under 18) and their parents/legal guardians. To minimize risk of injury, I agree to obey and/or instruct my registered students and their parents/legal guardians to obey all safety rules and report any physical problems to event staff immediately.2. Acknowledgement of Participation- By signing this waiver, I certify that (1) Myself (if over 18) and/or my registered students (if participants are under 18) are not currently under the care of a physician for any injury or illness that would prevent safe participation. (2) Myself and/or my registered students are not currently being treated for or recovering from any orthopedic injury that would prevent safe participation. (3) Myself and/or my registered students are in good health and able to safely participate in strenuous physical activity. (4) I will provide my and/or my registered student's health insurance provider and policy number upon request. (5) I have the consent of all registered students' parents/legal guardians to sign on their behalf if participant is under 18.3. Release of Liability - In consideration of my and/or my registered students (if participant is under 18) participation in a FLEUR event, I agree that (1) I am responsible for all costs associated with any injury of loss that myself and/or my registered students may sustain while participating in the event. (2) I certify that any and all registered participants carry health insurance that provides adequate coverage for any injuries or illnesses sustained during the event. (3) By signing below, I as well as all participants and their parents/legal guardians voluntarily and knowing release, discharge, and agree to hold harmless FLEUR Dance Convention, its owners, employees, agents, contractors, staff, venues, and affiliates, from any and all claims, demands, damages, or liabilities, including claims arising from ordinary negligence, except in cases of gross negligence or intentional misconduct. (4) This release applies to all claims of injury or damages except those resulting from gross negligence or intentional misconduct.4. Indemnity Clause - I as well as all participants and their parents/legal guardians agree to indemnify, defend, and hold harmless FLEUR Dance Convention and its employees, agents, and affiliates from and against any and all claims, liabilities, damages losses, and expenses (including reasonable attorney's fee(s) arising out of or related to my and/or my registered students (if participants are under 18) participation in a FLEUR event, including claims brought by my, my child, or any third party.5. Medical Treatment Authorization - I as well as all participants and their parents/legal guardians give permission for FLEUR and its employees to seek and obtain medical treatment for my and/or my registered students (if participants are under 18) in the event of an accident or illness during the event. I as well as all participants and their parents/legal guardians accept responsibility for any resulting medical expenses.6. Multi-State Governing Law - This waiver shall be governed by the laws of the State of Florida. However, any incident or claim arising from participation at a specific event shall also be subject to the laws of the state in which the event takes place. I as well as all participants and their parents/legal guardians agree to submit jurisdiction in Florida or the state where the event occurs.8. Acknowledgement - By completing registration and signing below, I certify that (1) I as well as all participants and their parents/legal guardians, have read and understand this Waiver & Release. (2) I as well as all participants and their parents/legal guardians voluntarily agree to all policies and terms stated above. (3) I am the legal studio owner/director have received consent from parent/legal guardians to sign on behalf of any and all of the registered participants if under 18. (4) I as well as all participants and their parents/legal guardians accept all risks associated with participation. (The full Terms & Conditions also apply).
