Camp Adventure Camper Registration 2026A Week-long Sleep-Away Camp Exclusively for Children with Cancer and Their Brothers and Sisters.

This application is to apply for Camp Adventure 2026 ONLY. However, children who meet the criteria for both camp experiences are welcome to fill out a seperate application for Saddle Rock Day Camp 2026. Please refer to the website for the application.

Camp Adventure Sleep-Away Camp is held from August 17th to August 23rd at Timber Lake West in Roscoe, NY.

Please read the eligibility requirements to be considered for Camp Adventure 2026 carefully before submitting your application:

Would the camper like to apply to either the Leader-in-Training (LIT) or the Counselor-in-Training (CIT) program at Camp Adventure?**Camp Adventure Leadership Program** Camp Adventure offers two leadership programs for teens and young adults: Leaders-in-Training (LIT) LITs are aged 17 (16 depending on availability) and over and assist in program activity areas while also having opportunities to just be a kid and have fun with friends. Counselors-in-Training (CIT) CITs are eligible when they are 18 and over and assist cabin counselors while also having opportunities to just be a kid and have fun with friends. **Eligibility Requirements** To be eligible for the LIT/CIT program, you must meet the following requirements: * Be at least 16 years old by the start of camp (LIT) or 18 years old by the start of camp (CIT). * Have a good academic standing * Provide a professional or academic reference * Have a positive attitude

HEALTH EXAM & PERMISSIONS

You are required to complete a physical health exam with your doctor and return this form within two weeks proper to the start of camp.

You can download the form after you submit this application. When you have completed it you can upload it to your application or return it via email.

Campers 18 and under must be up to date with MMR, and Varicella vaccinations and are required to get a PPD/TB Test within the guidelines outlined on the form.

Flu and COVID-19 Vaccinations are strongly recommended but not required.

Meningococcal Disease Risk - After reading, please select one option below* This field is required.Meningococcal disease is a potentially fatal bacterial infection commonly referred to as meningococcal meningitis. Under New York State Public Health Law (NYS PHL) §2167 and Subpart 7-2 of the State Sanitary Code, overnight children’s camps are required to distribute information about meningococcal disease and vaccination to all campers attending camp for seven (7) or more consecutive nights. Meningococcal disease is rare. However, when it strikes, its flu-like symptoms make diagnosis difficult. It can cause serious illnesses such as infection of the lining of the brain and spinal cord (meningitis) or blood infections (sepsis). The disease progresses quickly and can lead to severe, permanent disabilities — including hearing loss, brain damage, seizures, or limb amputation — in as many as one in five of those infected. Tragically, 10 to 15 percent of those who contract meningococcal disease will die. Meningococcal disease spreads easily through close personal contact, such as coughing, sharing beverages or eating utensils, kissing, or spending time in close quarters with someone who is sick or unknowingly carrying the bacteria. People can transmit the bacteria before showing any symptoms. While anyone can contract meningococcal disease, certain groups are at higher risk — including teens and young adults (ages 16–23) and individuals with specific medical conditions that affect the immune system. The single best way to protect against meningococcal disease is vaccination. The meningococcal ACWY (MenACWY) vaccine protects against four major strains of the bacteria that cause disease in the United States. The Centers for Disease Control and Prevention (CDC) recommends a first dose at age 11–12 with a booster at age 16. Children with certain underlying medical conditions may need to receive the MenACWY vaccine earlier. Additionally, the meningococcal B (MenB) vaccine protects against a fifth strain of the bacteria. Young adults (ages 16–23) may be vaccinated with MenB after consulting their healthcare provider. We encourage you to carefully review the attached Meningococcal Disease Fact Sheet, which is also available on the New York State Department of Health website. For information about the availability and cost of the vaccine, please contact your healthcare provider.

Medical Treatment & Insurance Responsibility I authorize camp staff to administer basic first aid and secure emergency medical care if needed. I understand that I am responsible for all medical expenses, and that my personal health insurance is the primary coverage. 4. Behavior, Dismissal & Refund Policy I understand that the Camp Director may dismiss any camper for behavior deemed unsafe, disruptive, or harmful to themselves or others. This includes, but is not limited to, violence, bullying, leaving camp grounds without permission, or possession of prohibited items. I understand that dismissal for behavior reasons is at the sole discretion of the Camp Director and that no refund will be issued. 5. Loss, Theft, or Damage to Property I understand that KiDS NEED MoRE, Inc. and its affiliates are not responsible for loss, theft, or damage to any personal property brought to camp, including but not limited to clothing, electronic devices, jewelry, sports equipment, glasses, musical instruments, or money. Any personal items brought to camp are at our own risk. 6. Health & Capability Acknowledgment I certify that my child is in good health and capable of participating fully in camp activities. I have disclosed any known medical conditions, behavioral concerns, allergies, or special needs that could impact participation.
