Fall 26 Preliminary Insurance Agreement

The international health insurance mandate must be fulfilled by international students for an entire academic year. Therefore, all international students automatically have an SI Hold placed on their account as needed.

In order to release the SI hold and be able to register during your scheduled date/time for Fall 2026, indicate how you intend to satisfy the mandate for the 2026-2027 academic year by completing/submitting this online form.

Please note: The USF Sponsored Student health insurance (SHIP) premium will automatically be assessed to your Student Self Service account when the hold is released pending the outcome of the option you choose below.

I am electing the USF Sponsored SHIP underwritten by Aetna Student Health for the 2026-2027 academic year (effective 8/17/26 to 8/16/27). Please select one of the options listed.

I will pay the premium for Annual or Fall coverage prior to Monday, August 24, 2026, via Student Self-Service. The 26-27 premium has not yet been determined. It will be automatically placed on my Student Self-Service account and Instructions will be sent to your USF email by July 15, 2026.I will be Department Sponsored and understand that I will be held responsible for the premium pending validation of my status from the department I select from the drop down menu below.I will be eligible for the Graduate Insurance Subsidy, as an employed GA/RA/TA for the Fall semester. I understand that I will be held responsible for the premium pending validation of my employment status from USF Human Resources.

To be eligible for the Fall GA insurance subsidy program, I understand that I must meet the following:*

Be an active employee in the USF employment system in one of the following job codes: 9181, 9182, 9183, 9184, 9185, or 9550 and work a minimum of 10 hours a week (.25 FTE of higher)

I understand the following next steps:*

1) I must successfully complete the pre-boarding process and receive job offer letter. 2) Monitor your emails starting on July 15, 2026, for an email frominfo4students@mycare26.com.3) Click on the link in the email to activate account and enroll for the GA/RA/TA Subsidy program by August 24, 2026.

I am electing to waive/opt out of the USF Sponsored SHIP and will provide proof of an adequate alternative health insurance policy for the 2026-2027academic year (8/17/26-8/16/27).I understand that I must maintain continuous coverage and will be held responsible for the annual SHIP premium until my alternative coverage is validated. Returning students cannot have a gap in coverage.

I am providing proof of an alternative health insurance policy purchased by me and/or a family member and understand that I will be held responsible for the SHIP premium pending validation that the policy submitted is adequate.No coverage should be purchased for the 2026/2027 Academic Year until after July 15, 2026. We will request receipts reflecting date of purchase.I am a Sponsored/Funded International Student and will provide proof of my enrollment in an alternative health insurance policy provided by a NON-USF Organization.I have DACA Status and I will supply my current Notice of Action I-797.

F/J visa holder:The Health Insurance REQUIREMENTS HAVE CHANGED. I will not buy an alternative policy for the 26/27 academic year until after July 15th and I have read the updated insurance regulations. Instructions will be sent to your USF email by July 15,2026. Waiver/Opting out documents submission dates is from July 15, 2026 to August 24, 2026 and I understand the following:*

If I miss the final deadline of August 24, 2026, I will be required to purchase Fall coverage under the USF Sponsored SHIP.

I understand that I will need to provide my current Financial Guarantee or Scholarship award letter, as well as a copy of the front and back of my health insurance card. I will select my sponsoring organization from the menu below.
