- Families typically apply with a specific activity in mind. Activities must be organized and structured, and provide a positive experience for the child.
- Activity provider contact information (coach, program director, front desk, or billing contact)
- Estimated program or activity costs
Child’s School Enrollment Verification

Please upload a report card or official school document that shows the child’s name and school to verify K-12 enrollment in Arizona.

All applicants must be enrolled in a K-12 school in Arizona. If the child is not enrolled in a traditional school, you must provide a state-approved educational plan or documentation outlining the child’s alternative education program (such as homeschool or other approved program).

Household Income Documentation

We must verify your household’s total income to review your application. Please have one (or a combination) of the following ready:
- Most recent federal tax return (IRS Form 1040) (preferred)
- Three months of pay stubs from each working parent or guardian
- Government assistance documentation showing program start and end or renewal dates (such as SNAP, TANF, SSI, WIC, etc.)
All applicants must come from households with income that falls at or below 150% of the Federal Poverty Guidelines. Please review the table above. Meeting the income requirement does not guarantee approval.

Please complete all required fields and upload the requested documents to avoid delays in review. When in doubt, it is best to provide more information rather than less. Applications submitted with missing information may be denied. Applications are reviewed by the Opportunity4Kids Scholarship Committee in the order received.

Scholarship Payment Policy

Opportunity4Kids scholarships are reviewed and approved by the Scholarship Committee. If approved, scholarship funds are paid directly to the activity provider and are not distributed to families. Opportunity4Kids will coordinate payment directly with the activity provider using the contact information supplied in this application.

Liability Waiver

It is expressly understood that Opportunity4Kids is not liable for any accidents or injuries which may occur while the child is participating in the chosen activity. As a condition of accepting a scholarship provided by Opportunity4Kids, the participant specifically waives, releases, and discharges any and all potential claims for injuries or damages and releases and agrees to hold harmless Opportunity4Kids, its Board Members, trustees, officers, employees, and volunteers from and against any and all liability arising out of or connected in any way with participation in the activity.
