The law requires that parental permission be obtained for medical procedures performed on minors (under 18). The parents/legal guardians should initial the following consent form so that such procedures can be promptly carried out. We will notify you in case of an illness or emergency. In the event of a serious illness or injury, we will transport and accompany students to the Sisters of Mercy Urgent Care Unit or, as appropriate, to Mission Hospital.
I, the undersigned parent/guardian hereby give permission to the staff of the Teacher Academy camps to have my son/daughter referred to an off-campus physician in the event it becomes necessary.
Photo Release: You agree for photos of your student to be used on Mars Hill University official or affiliated website, social media and advertising.
