If you plan to register for both days you must complete each individual form and pay.

There will be no refunds if you decide not to show.

Level 3 competitors MUST pre-register per trial day a minimum of 4 weeks in advance. If we receive more than 20 entries prior to the 4 weeks, then there will be no spots open for a Level 3 entry.

NOTE: You must be a member of PSA to show in the trial. You may join PSA the day of the trial.

REMEMBER YOUR SCOREBOOK.

If you need a new scorebook, you may purchase one via the PSA website leading up to/the day of trial. Proof of purchase must be shown at check in.

Acknowledgment of Bylaws and Code of Conduct
By signing below, I affirm that:

1. I have reviewed and understand the current PSA–AS Bylaws and Code of Conduct as published on the PSA-AS website as of the date of signature.
2. I agree to abide by all rules, regulations, and standards of conduct set forth in these documents.
3. I understand that compliance with the Bylaws and Code of Conduct is a condition of my membership and/or participation in any sanctioned PSA–AS event.
4. I acknowledge that any violation of these rules may result in disciplinary action, up to and including suspension, revocation of membership, or a lifetime ban, in accordance with the disciplinary procedures outlined in the PSA–AS Bylaws and Code of Conduct.
5. I understand that PSA–AS reserves the right to amend or update its Bylaws and Code of Conduct, and that it is my responsibility to remain informed about any such changes.
6. I understand that PSA–AS reserves the right to amend or update its Bylaws and Code of Conduct. I acknowledge it is my obligation to stay informed about any changes to these documents, which will be made available via the official PSA–AS website and/or the official PSA–AS Facebook group page.
 

RELEASE: It is the understanding that the above handler / dog team are entering this trial AT THEIR OWN RISK. I understand that serious accidents can and do occur during dog training, competitions and other dog related events / activities. I am assuming all risk, liabilities and responsibilities and will hold harmless PSA K9, their agents, partners, employees, sponsors from ANY AND ALL liability arising from this dog-related event and practice training. I also agree to hold harmless any of the above mentioned from any property damage, theft, death or personal injury to my dog or myself. I also agree to have complete control over myself and my dog(s) at all times. By signing this entry form I acknowledge that I have read and understood the trial waiver and read and understand all PSA rules that govern trials. There will be no refunds given to any handler that has been ejected from this trial for any reason. In addition, by signing this release I give consent to any representative of PSA or Tarheel Canine Inc., to take pictures of events and any activities surrounding the events. Further by submitting this form, I release any rights to the sale, posting or distribution of any pictures taken by Tarheel Canine and waive any right to remuneration or compensation for any picture taken or sold of the events in question.
