• DBT Skills Group Informed Consent

  • Practice Name: Liberated Love Counseling LLC
    Facilitator: Rae Fuqua, MFTC
    Group Format: DBT Skills Group
    Length: 30 weekly sessions

Purpose of the DBT Skills Group

Dialectical Behavior Therapy (DBT) skills groups are designed to help participants learn and practice skills for improving emotional regulation, distress tolerance, interpersonal effectiveness, and mindfulness.

This group focuses on learning and practicing skills, rather than processing individual therapy concerns in depth. While supportive discussion may occur, the primary goal is education and skill-building.

Participation in the group does not replace individual therapy, and participants may be encouraged to maintain an individual therapist during the program.

    Group Structure

    • The group meets once per week for approximately 90 minutes.
    • Sessions will include skills teaching, discussion, and skills practice.
    • Participants are highly encouraged to practice skills between sessions.

    Confidentiality

Your privacy is important and will be respected.

The group facilitator is legally and ethically required to maintain confidentiality except in situations required by law, such as:

    • Risk of harm to yourself
    • Risk of harm to others
    • Suspected abuse or neglect of a minor, elder, or dependent adult
    • Court order

However, confidentiality in a group setting cannot be absolutely guaranteed, as other group members are not legally bound by therapist confidentiality laws.

All participants will be asked to respect the privacy of others and not disclose personal information shared during group sessions outside of the group. All participants will also be asked not to contact each other outside of the group unless clear consent is expressed by all parties.

    Group Participation Expectations

Group therapy works best when participants are actively engaged.

    Participants are asked to:

    • Attend sessions consistently
    • Arrive on time
    • Participate respectfully
    • Allow space for all group members to speak
    • Practice skills outside of sessions when possible
    • Participants are not required to share personal details beyond what feels comfortable.

    Fees, Attendance and Missed Sessions

The total cost for the DBT skills group is $1500 for the full 30-session program. Your enrollment reserves a spot in the group for the full program cycle. You can either pay the fee in full, or, you can pay in installments through Klarna or Affirm.

Because space in therapy groups is limited:

    • Missed sessions are not refunded
    • Participants are responsible for payment regardless of attendance
    • Materials may be provided for missed sessions when possible

If you anticipate missing multiple sessions, please discuss this with the facilitator.

Possible Emotional Impact and Support Outside of Group

    Possible benefits include:

    • Increased emotional regulation
    • Improved relationships
    • Greater self-awareness
    • Stronger coping skills

Participating in a skills group may sometimes bring up difficult emotions or experiences. Participants are encouraged to communicate with the facilitator if group participation becomes distressing.

If you need extra support outside of group sessions, you are encouraged to text your facilitator, who will respond within 2 hours during reasonable day and evening time hours. Calls are provided as available and are limited to 15 minutes. During the call/text session, you will be guided through the skills that have been covered so far in the group. This service is not to be used for crisis situations that require immediate assistance.

    Crisis and Emergency Support

This DBT group does not provide crisis services.

If you experience a mental health emergency, please contact:

    • 911 for Medical Emergencies
    • 988 for the Suicide & Crisis Lifeline
    • Local emergency services (can be provided upon request)

Participants are encouraged to maintain outside support resources while participating in the group.

    Voluntary Participation

Participation in this group is voluntary. You may withdraw from the group at any time. If you are withdrawing due to an unforseen medical or family emergency, please contact your facilitator for refund options. Otherwise, you will not receive a refund for withdrawal.

The facilitator may also recommend discontinuing group participation if it appears that the group is not an appropriate level of care. If you discontinue the group due to the facilitator's recommendation, you will be refunded at a prorated rate based on the number of sessions you have attended.
