I, the undersigned participant or parent/legal guardian of the participant listed above, acknowledge that participation in activities at Compassion at Calvary Church and its events, including but not limited to the Easter Egg Hunt, may involve physical activity and interaction with others and may carry a risk of injury. By signing this form, I voluntarily assume all risks and agree that Compassion at Calvary Church, its pastors, staff, volunteers, and representatives shall not be held liable for any injury, illness, accident, or damages that may occur while participating in the event or while on church property. I also understand that Compassion at Calvary Church is not responsible for any lost, stolen, or damaged personal property brought to the event. In the event of an emergency involving a minor participant, I authorize church staff or volunteers to seek or obtain medical treatment if I cannot be reached immediately, and I accept responsibility for any medical expenses incurred. Additionally, I grant permission to Compassion at Calvary Church to photograph or record video of the participant during the event and authorize the use of these images or recordings for church-related purposes including but not limited to social media, website content, promotional materials, and church publications without compensation. By signing below, I acknowledge that I have read, understood, and agree to the terms of this waiver and release.
