NKT Level 1 Registration Required

MMT is a pretraining course for NKT Level 1. You must be registered for an upcoming NKT Level 1 course before enrolling in MMT.

Please complete your NKT Level 1 registration first. Once you are registered, you can return to this form to complete your MMT enrollment.

Please review this policy carefully. By registering for the MMT Course, you acknowledge and agree to the terms below.
 
- MMT and NKT are separate organizations.
- Each class requires a separate registration and payment.
- Cancelling an MMT registration does not cancel an NKT seminar registration.
- Cancelling an NKT registration does not cancel an MMT registration.
- Students registered for both classes must contact each organization separately    regarding cancellations, transfers, and refunds.

Media Release and Consent for Educational and Marketing Purposes

I understand that photographs, video recordings, audio recordings, and other media may be captured during the MMT Course for educational, promotional, and marketing purposes.

Media Consent Release:

I grant permission to MMT, Body Moxie, LLC, Christine Wue, Lynnette Yasukawa, and their authorized representatives to use photographs, videos, audio recordings, testimonials, and other media containing my image, likeness, voice, or participation in connection with:

- Educational materials and course resources
- Online learning platforms and course promotion
- Websites, newsletters, social media, and other digital marketing channels
- Presentations, workshops, and promotional materials

I understand that my name and identifying information will not be publicly shared without my additional permission. I waive any right to inspect or approve the finished materials and release MMT, Body Moxie, LLC, instructors, and representatives from any claims related to the use of approved media.

I understand that participation in photographs, videos, or recordings is voluntary and that I may decline media participation without affecting my ability to attend or participate in the Course.

 

Intellectual Property & Confidentiality
MMT courses include proprietary educational materials, information, techniques, methods, demonstrations, terminology, and curriculum developed or owned by MMT and/or its instructors. These materials are provided to participants solely for their personal education and use in connection with the course.

By participating in an MMT course, I acknowledge and agree that:

All course materials, including but not limited to manuals, handouts, presentations, diagrams, videos, worksheets, recordings, written materials, and other educational content, remain the intellectual property of MMT and/or their respective owners.

I will not copy, reproduce, distribute, publish, sell, sublicense, upload, or otherwise share MMT course materials, in whole or in part, without prior written permission.

I will not photograph, audio-record, video-record, or otherwise record course instruction, demonstrations, presentations, or materials without prior permission from MMT.

I will not upload MMT course materials, recordings, photographs, or other proprietary course content to AI tools, online platforms, file-sharing services, or other third-party services without prior written permission.

I will not use MMT proprietary curriculum or course materials to create, develop, teach, market, or sell a competing course, training program, publication, or other educational product without prior written permission from MMT.

I understand that participation in an MMT course does not transfer ownership of any MMT intellectual property to me.

I may use the general knowledge, skills, and techniques I learn through the course in my own professional practice, provided that I do not copy, reproduce, distribute, or present MMT proprietary materials or curriculum as my own.

I agree to respect the confidentiality of any proprietary or confidential information shared during the course and will not disclose such information to individuals who are not authorized to receive it.

I understand that these provisions are intended to protect MMT's intellectual property and proprietary educational content while allowing participants to use the knowledge and skills they acquire through the course in their professional practice.

 

MMT Course Waiver and Cancellation Policy

By submitting this form, I confirm that I am registering for a Manual Muscle Testing (MMT) course (the “Course”) and agree to the terms outlined below.

1. Acknowledgment of Physical Demands
I understand that this Course involves hands-on movement activities with other participants. These activities may be physically demanding, and participants may have varying levels of expertise, experience, and physical ability in applying the techniques taught.

2. Voluntary Participation
My participation in this Course is entirely voluntary. I agree to fully engage in all aspects of the Course and will disclose any pre-existing conditions or injuries to the instructors before the start of the Course. I understand that such conditions may limit my ability to participate fully.

3. Assumption of Risk
I understand that the Course involves physical contact and hands-on application, which may carry inherent risks, including physical injury. I voluntarily assume all risks and agree to participate at my own discretion and responsibility. I will follow all safety instructions and act with care at all times.

4. Release of Liability
I, for myself and on behalf of my heirs, personal representatives, and assigns, release, discharge, and hold harmless Christine Wue, Lynnette Yasukawa, Body Moxie, LLC, instructors, organizers, volunteers, venue hosts, and any affiliated agents or representatives (the “Released Parties”) from any and all claims, demands, or suits arising from my participation in the Course, whether caused by negligence or otherwise.

5. Hold Harmless and Indemnification
I agree to indemnify, defend, and hold harmless the Released Parties from any claims, damages, liabilities, or expenses (including attorneys’ fees) arising from my participation in the Course, including any injuries or losses I may sustain.

6. Medical Treatment
In the event of injury or medical issues, I authorize Course organizers to seek medical assistance on my behalf. I accept full responsibility for any associated costs and will inform organizers of any medical conditions beforehand.

7. Agreement to Hold Harmless
I further agree to hold harmless MMT, Body Moxie, LLC, Christine Wue, Lynnette Yasukawa, instructors, organizers, and participants from any injury, illness, or damage I may sustain in connection with my participation.

8. Understanding and Acceptance
I confirm that I have read, understood, and agree to all terms in this document. Any questions I had were answered to my satisfaction. This Release and Waiver applies to all MMT courses in which I participate.

9. Dispute Resolution
Any dispute arising from this Agreement or my participation in the Course will be resolved through binding arbitration under the laws of the state where the Course is held.

10. Cancellation, Transfer & Refund Policy
Cancellation Due to Low Enrollment
If a course is canceled due to insufficient enrollment, I will receive a full refund of all registration fees paid. No administrative fee will be applied.

Participant Cancellations, Transfers, and Refunds

More than 14 days before the course date: I may request either a refund, less a $25 non-refundable administrative fee, or transfer to another MMT course within one (1) year of the original date, subject to a $25 administrative fee.

14 days or fewer before the course date: No refund will be issued, but I may transfer my registration to another MMT course within one (1) year.
Same-day cancellations or no-shows: No refund or transfer will be granted; registration is forfeited.

Instructor Illness or Travel Disruptions
In the event of instructor illness or unforeseen travel disruptions (including weather delays), the course will be rescheduled. I may attend the rescheduled course or transfer to the same course at a different location or date. No additional compensation will be provided.

Force Majeure
In case of circumstances beyond the organizers’ control (natural disasters, pandemics, political unrest, terrorism, labor strikes, transportation shutdowns), a full refund may not be possible. I may transfer to another MMT course within one (1) year, or a refund will be issued less the $25 non-refundable administrative fee.

Travel and Accommodation Disclaimer
I acknowledge that I have been advised not to book travel or lodging until the course is confirmed and minimum enrollment is met. I understand that MMT and its agents are not responsible for any travel, lodging, or transaction-related expenses due to postponement, cancellation, or rescheduling. 

 

Acknowledgment and Consent
By signing below, I confirm that I have read, understood, and agree to all terms in this document, including the waiver and cancellation policy. I affirm that I am 18 years of age or older and that I am signing this voluntarily.
