Without a completed permission form, the booking will not be confirmed and your child may miss out on attending the excursion.
Excursion bookings will not be approved until we receive the excursion permission form.
An excursion permission form provides your authorisation that your child/ren are able to be taken on an excursion away from the service premises. A risk assessment of this excursion has been prepared and is available at the service should you wish to view it.
