    • SECTION 1: PERSONAL INFORMATION
    • Format: (000) 000-0000.
    • MPIRMC serves members across a five-state region, and we are committed to building a board that reflects the geographic diversity of our community. Your location is not a factor in eligibility — we welcome and encourage applicants from across Colorado and our out-of-state membership base to apply.

    • SECTION 2: MPI MEMBERSHIP & INVOLVEMENT
    • Membership Eligibility Confirmation

To serve on the MPIRMC Board of Directors, you must be an MPI member in good standing with your primary chapter affiliation listed as Rocky Mountain.

By continuing this application, you confirm that your MPI membership is active and will not expire before April 30, 2026. Members whose membership has lapsed or is inactive as of the application deadline may not be eligible for consideration.

If your membership is approaching renewal, we encourage you to renew before submitting to avoid any eligibility concerns. If renewal presents a financial hardship, MPI may offer assistance programs or scholarships — reach out to MPI directly as soon as possible.

    • 2026–2027 Board Term — Key Date Availability

    • Please review the key dates for the 2026–2027 board term and indicate your availability for each.

Having a conflict will not automatically disqualify you, but availability will be considered during the nominations process and may be discussed during your interview.

    • SECTION 5: INDUSTRY INVOLVEMENT & DISCLOSURES
    • The following questions are intended to ensure transparency and fairness throughout the nominations process. Disclosure of any relationship or affiliation does not disqualify an applicant — it simply allows the Nominating Committee to take appropriate steps such as recusal where necessary.

    • SECTION 6: YOUR LEADERSHIP JOURNEY
    • This section helps the Nominating Committee understand not just where you are today, but what drives you and where you're headed. There are no right or wrong answers — we want to hear your authentic voice.

How would you rate your current readiness in the following areas?

These ratings are not used to disqualify candidates. They help the Nominating Committee identify the best role alignment and meaningful leadership development opportunities for each applicant. Please be honest — there are no wrong answers here

    • SECTION 7: COMMITMENT, SIGNATURE & SUBMISSION
    • Serving on the MPIRMC Board of Directors is one of the most rewarding ways to invest in your industry and our community — and it is a genuine commitment. This is a working board, which means we expect more than a title. Board members are expected to show up, contribute, collaborate, and follow through.

Beyond attending meetings and events, you will be asked to take ownership of initiatives, support your fellow board members, and actively participate in the work that keeps this chapter moving forward. We are a team in the truest sense — and every seat at the table matters.

If life gets busy (and it will), we ask that you communicate early and often rather than go silent. What we ask for above all else is presence, effort, and accountability.

By submitting this application and signing below, you confirm that you understand and embrace what board service with MPIRMC truly looks like.

Your signature below serves as your commitment — to this application, to this process, and to the standards of board service described above. It confirms that all information provided is true, accurate, and complete to the best of your knowledge, that you are submitting this application of your own accord, and that you fully understand and accept the expectations that come with it. Any intentional misrepresentation of information may result in immediate disqualification from the nominations process or removal from the board if discovered after placement.
