Waiver of Liability and Photo Release
By registering for, attending, or participating in any class, practice, event, camp, competition, or activity hosted by Elevation Cheer & Dance Company, participants and/or their parent or legal guardian acknowledge and agree to the following:
1. Assumption of Risk
Participation in cheerleading, dance, tumbling, and related activities involves physical activity and inherent risks, including but not limited to falls, collisions, sprains, strains, or other injuries. By participating, you voluntarily assume all risks associated with these activities.
2. Release of Liability
Participants and/or their parent or legal guardian agree to release, waive, and hold harmless Elevation Cheer & Dance Company, its owners, directors, coaches, staff, volunteers, and affiliates from any and all liability, claims, demands, or causes of action arising from or related to participation in any Elevation Cheer & Dance Company activities, to the fullest extent permitted by law.
3. Medical Responsibility
Participants confirm that they are physically able to participate in cheer and dance activities. In the event of an injury or medical emergency, Elevation Cheer & Dance Company staff may seek appropriate medical treatment if a parent or guardian cannot be reached. Any medical expenses incurred are the responsibility of the participant or their parent/guardian.
4. Photo and Video Release
Elevation Cheer & Dance Company may take photographs or video recordings during classes, practices, performances, or events. By participating, you grant Elevation Cheer & Dance Company permission to use these images or recordings for promotional, marketing, social media, and educational purposes without compensation.
5. Acknowledgment of Agreement
Participation in any Elevation Cheer & Dance Company program constitutes acknowledgment and acceptance of this Waiver of Liability and Photo Release.
If you do not agree to these terms, please do not participate in Elevation Cheer & Dance Company activities.
