Thank you for taking the time to nominate and recognise someone for an England Netball Centenary ONE Award. Please make sure you have read the guidance document which details the awards and criteria as well as some tops tips for completing your nomination form. The nomination form is broken down into 4 parts some areas are mandatory, and some have a maximum word count, so make sure you follow the guidance.

Some Regions have their own regional awards, you may be directed to a 5th Part if your region has these questions.

Before you complete your nomination form please make sure you have the following information to hand, as this will assist you in being able to complete all sections. You will be able to return to the form if you need to at a later date, but you must complete section 1 in order to do this: 

• Contact details for the person you are nominating - email, telephone and if you can their ENA ID (membership number) 
• Consent from the person you are nominating to share their details 
• Up to 3 quotes (max 100 words each) to evidence the impact they have had on others  

Please note when submitting a nomination and selecting both National and Regional this will be for the same person. You can NOT nominate 2 different people in one submission you will need to complete 2 separate forms. 

Questions that have a * are mandatory fields, all other questions are recommended to support and strengthen your nomination form. 

East Midlands Jean Perkins Walking Netball Award
The Jean Perkins Walking Netball Award is presented to a Walking Netball participant from the East Midlands whose personal lifestyle has changed significantly since coming to the Walking Netball sessions.

East Midlands Jean Bourne Back to Netball Award

The Jean Bourne Back to Netball Award is presented to a Back to Netball participant from the East Midlands whose personal lifestyle has changed significantly since returning to netball.

East Midlands Jean Staley Award
This award was initiated in memory of Jean Staley, recognising her commitment to administration, playing and umpiring across Club, County and Region.

East Midlands Long Service Award
Approved by the East Midlands Regional Management Board for any registered participant or affiliated school member who has given at least 10 years of service (not necessarily consecutive) to the Region in any capacity (Player, Umpire, Coach, Administrator, Volunteer, Table Official, etc.).

East Midlands Jean Perkins Walking Netball Award
The Jean Perkins Walking Netball Award is presented to a Walking Netball participant from the East Midlands whose personal lifestyle has changed significantly since coming to the Walking Netball sessions.

• Nominees must have taken part in an East Midlands Walking Netball session and completed an England Netball Participant Registration Form (this has been checked; any not completed have been removed).
• Nominations should demonstrate how Walking Netball has positively impacted the participant’s lifestyle, as well as showing their passion, support and dedication to their group (e.g., organising social events, becoming a WN Host, organising special tops/T‑shirts).

East Midlands Jean Bourne Back to Netball Award
The Jean Bourne Back to Netball Award is presented to a Back to Netball participant from the East Midlands whose personal lifestyle has changed significantly since returning to netball.

• Nominees must have taken part in an East Midlands Back to Netball session and completed an England Netball Back to Netball Participant Registration Form (this has been checked).
• Nominations should demonstrate how Back to Netball has positively impacted the participant’s lifestyle, as well as something special they have done or achieved for their group (e.g., organising social events, completing an officiating course, looking after kit).

East Midlands Jean Staley Award
This award was initiated in memory of Jean Staley, recognising her commitment to administration, playing and umpiring across Club, County and Region.

• The nominee should have been involved at Club, County and Region in one or more of the following roles: Administration, Umpire or Coach. The emphasis is on work done at all levels (not necessarily in the same discipline).

East Midlands Long Service Award
Approved by the East Midlands Regional Management Board for any registered participant or affiliated school member who has given at least 10 years of service (not necessarily consecutive) to the Region in any capacity (Player, Umpire, Coach, Administrator, Volunteer, Table Official, etc.).

Netball South West Long Service Award

Criteria:

• Volunteers must be a member of Netball South West.
• Evidence of commitment at county and/or regional level (not just at club level) for at least 25 years within any volunteer capacity e.g Umpire, Coach, Administrator, Volunteer, Table Official.
• Self-nomination is permitted.
• Service need not be concurrent.
• All nominations must be seconded by the County or Regional Association.

Netball South Long Service Award
Approved by the Netball South Regional Management Board for any registered participant or affiliated school member who has given at least 10 years of service (not necessarily consecutive) to the Region in any capacity (Player, Umpire, Coach, Administrator, Volunteer, Table Official, etc.) and at any level (local, count, regional).
