Liability Waiver & Agreement

This waiver is valid for 12 months from the date signed and applies to all training activities at Legend Sports Court during that time.

1. Assumption of Risk I understand that participation in basketball training and related activities involves inherent risks of injury, including but not limited to falls, contact with other participants, and physical exertion. I voluntarily assume all risks on behalf of myself and/or my child.

2. Release of Liability I hereby release and hold harmless Legend Sports Court, its owners, staff, and coaches from any and all liability for injuries, damages, or losses that may occur as a result of participation in training activities at the facility.

3. Medical I understand that Legend Sports Court does not provide medical services. In the event of an injury, I authorize staff to contact emergency services if necessary and accept full responsibility for any medical expenses.

4. Supervision Parents/guardians are responsible for their child before and after class. During training, children are under coach's supervision only for the duration of the class.

5. Media Consent I grant permission for Legend Sports Court to use photos or videos of training sessions for promotional purposes unless I notify staff otherwise in writing.

6. Payment Policy I understand that class payments are non-refundable and that payment is required to secure a spot in the program.
