Your child has been diagnosed as having Attention Deficit Hyperactivity Disorder (ADHD).
Medications used to treat ADHD are tightly regulated by both state and federal law. By signing this form, I acknowledge I have read the ADHD agreement, I understand, and agree to the conditions:
- Common side effects of ADHD medications are loss of appetite, change in sleep schedule,and moodiness when the medication is wearing off.
- Call us immediately to set up an appointment if your child has any side effects you think are serious.
- After medication is started a number of follow-up appointments will be required until the doses are correct. An appointment will be required 30 days after initiation of medication. After that time, your child will usually need to be seen every 3 months, or sooner if warranted. No prescriptions will be given if appointments are not kept.
- If you think your child needs a dosage change, schedule an appointment.
- After any dosage change, a follow-up visit is usually needed before any refill is given.
- ADHD prescriptions are written for a 30-day supply. By law, no refills can be put on the prescription. You must call each month for a refill.
- To request a refill, call our prescription line well in advance. You need to allow at least 2 business days for any prescription refill.
- ADHD prescriptions are sent to your pharmacy electronically.
- ADHD medications are for your child only. Any suspect misuse or abuse of this medication should be reported immediately.
- Grace Health should be called immediately if a prescription or prescribed medication is lost or stolen. We may not be able to replace the missing item.
- Random drug testing may be required.
