All applicants are required to submit the following supporting documents:
1. A completed and signed Proof of Diagnosis Form from your treating health care professional.
2. A signed and dated Applicant Consent Form.
3. Two (2) letters of personal recommendation from a teacher, guidance counsellor, school official, coach, mentor, health care provider, professional colleague, or employer.
4. A one (1) page (500-word max.) essay outlining how you demonstrate your academic aspiration and strive to sustain an optimal level of wellness, allowing you to excel in reaching your personal and academic goals and inspiring others to do the same.

Clinton Shard Memorial Honour

Please read the section below and complete the questions if you would like to be considered for the AbbVie IBD Clinton Shard Memorial Honour.
As of 2022, the AbbVie IBD Scholarship program awards one (1) of the fifteen (15) recipients with the Clinton Shard Memorial Honour, along with the $5,000 scholarship funds. Please note this is an honour, and not an additional scholarship.

The recipient will be selected by Clinton's family to honour the memory and impact of Clinton Shard. Learn more about Clinton Shard and his award.

The recipient of this honour is selected based on the following criteria:
- Studies, volunteers, and/or embodies a passion for the environment such as environmental sustainability/protection/rehabilitation or;
- Studies, volunteers, and/or embodies a passion for the arts or;
- Displays a commitment to their community through volunteer work and/or;
- Has been presented with or had to overcome economic, social, emotional or other hardship(s) as a result of or in addition to their IBD diagnosis.
