Coaching Agreement & Client Consent Form

This form is your official coaching agreement. It outlines the structure of our work together, including the monthly retainer, session expectations, boundaries, and confidentiality.

Please read through each section carefully and check the boxes and sign at the end to confirm your understanding and agreement.

This space is built on trust, clarity, and mutual respect and this agreement is here to support that foundation.

You are enrolling in:

Foundations + Freedom | Tier 3

This program is a 12-month coaching retainer combining private mentorship, group coaching, and structured curriculum.

Private Coaching is monthly retainer-based – not per-session.

Your retainer reserves your space in Renee's private client roster and includes live sessions and support behind the scenes. Unused sessions do not roll over and there are no refunds or credits unless otherwise agreed in writing. 

Your retainer includes:

• $985 per month for 12 months
• One (1) private Zoom session per month
• Two (2) monthly group coaching Zoom sessions
• Access to the Foundations & Freedom curriculum
• Access to the private Skool community
• 12-month minimum commitment
• Access to Signal (app) for Private communication with Renee between sessions

Payment is due prior to the start of the program.

By enrolling, you authorize automatic monthly payments via Stripe.

This is a 12-month commitment. Your 12-month commitment begins on the date your first payment is received.

Payment plans are not subscriptions and cannot be canceled early. 

If a scheduled payment fails or is declined, we'll attempt to process it again and will reach out to update your payment method if needed. If a payment remains unresolved for more than 10 days, your sessions and access to program materials may be paused until payment is received. Pausing access does not change your total commitment — you remain responsible for all remaining payments under your 12-month commitment, and unpaid amounts may be sent to collections if we're unable to resolve the issue together.

Because you receive immediate access to proprietary curriculum and materials, all payments are final and non-refundable.

Continuation: 
As your 12-month commitment nears its close, we'll reach out about 15–30 days ahead to talk about your next chapter with Renee, whether that's continuing with a new 12-month commitment, moving to month-to-month coaching, or wrapping up. After we talk, we'll send a written confirmation of what we agreed to, and we'll ask you to let us know at support@reneejayne.com within 5 business days if anything doesn't match your understanding. Your billing will simply follow whatever we've confirmed together, and we won't continue billing you without that confirmation.

Practitioner Availability:
In the rare event that Renee is unable to continue coaching during your 12-month commitment due to illness, injury, or other unforeseen circumstances, we will work with you to either pause your program until she's able to resume, offer sessions with another qualified practitioner, or arrange a prorated resolution for the remaining term. Your commitment and payment schedule will not be forfeited without a resolution being offered.

Sessions cannot be canceled for a refund.

If you need to reschedule a private session, please provide at least 24 hours notice by contacting: support@reneejayne.com

If less than 24 hours notice is given, the session may be forfeited.

Group calls cannot be rescheduled.

Some sessions may be recorded (upon request) and shared within the program platform.

Recordings are available for a limited time during the program.

 

Please show up fully present in a quiet, private space. Make sure you are free to speak without being heard by family or friends. Avoid driving, eating, or multitasking during sessions. Have something available for taking notes. While Renee may occasionally share notes or resources, you are encouraged to take your own notes.

Everything shared in sessions remains confidential unless you provide written permission or disclosure is legally required.

Within the group container, all participants are expected to respect the confidentiality of others.

This is a safe space for honest exploration. If something feels off for you, please raise it so we can address it together. This relationship is designed to support open and conscious communication.

All program materials, teachings, recordings, and curriculum are the intellectual property of Renee Jayne Wellness.

They may not be copied, shared, distributed, or taught without written permission.

All materials provided within this program are informational and educational only and are not medical or mental health advice.

Please consult your physician before making dietary or lifestyle changes, especially if pregnant, nursing, taking medication, or managing a medical condition.

Participation in any cleanse is voluntary and undertaken at your own discretion.

 

Coaching is a collaborative process that requires active participation.

Results vary based on many factors including personal commitment, effort, life circumstances, and individual implementation.

No guarantees are made regarding specific outcomes.

By enrolling in this program, you agree to communicate directly with Renee Jayne Wellness at support@reneejayne.com to resolve any concerns prior to initiating a payment dispute or chargeback with your financial institution.

Submitting a chargeback after agreeing to this contract may be considered a breach of this agreement.

This agreement is entered into by Que Sera Media, LLC (d/b/a Renee Jayne), and is governed by the laws of the State of Michigan, without regard to conflict of law principles. Any disputes arising from this agreement will be resolved in the state or federal courts located in Oakland County, Michigan.
