Agreement:
The undersigned parents or legal guardians/custodians (collectively, “Parents”) of the below identified child (“Child”) hereby acknowledge and agree to the terms and conditions identified below, for Child to participate in a program for bus transportation to and from Paul Revere Charter Middle School (“Paul Revere”) for the 2026-2027 school year coordinated by PRMS Student Services, Inc. (the “Program”), as described herein.
Parents agree to pay $3,150.00 (subject to change) for transportation of Child to and from Paul Revere from the first day of the school year yet to be released by LAUSD but estimated to be August 12, 2026 through the last day of the school year estimated to be June 4, 2027 (“Program Term”). Such payment shall be made in full by May 31, 2026 by:
- Credit/Debit Online Payment (3.5% per seat transaction fees will be applied at check out)
- Avoid the fees and pay by check, payable to PRMS Student Services, Inc., dropped off in addressed envelope to the attention of Lori Vogel, PRMS Student Services, Inc. at Paul Revere's Main Office by June 12, 2026 or mail to:

Child will be assigned to a route with the stop requested, but not guaranteed to be on a specific route (such as with another specific student), except that we will always assign siblings to the same bus. All buses shall be configured with three passengers per side. All students will be assigned seats by the driver at the driver's sole discretion.
NO REFUNDS OF ANY KIND once payment is made and seat availability confirmed by PRMS Student Services, Inc., except when a route can't be filled and needs to be cancelled. If your route is full at the time of payment, you will be placed on a wait list and contacted should a seat become available at a later date.
