I understand that Prosperity Ascend Group LLC will review my credit information and prepare administrative dispute documentation based on the items previously discussed during my credit review.

Only the accounts and items discussed and included in my selected service package will be addressed.

Any additional accounts, collections, inquiries, or negative items that appear after the start of services are not automatically included in the original service package and may require additional review or service fees.

All information submitted through this form is encrypted and kept strictly confidential. Documents are used solely for the purpose of preparing credit dispute documentation.
