Important information:

The Tee Party Modeling Showcase is happening April 25th at Calvary Church located at 274 W. Glengate Ave in Chicago Heights, IL.  Only 25 models may participate.  We will select a queen for each scene and crown an overall TOP MODEL at the end of the event! 

Models are responsible for their own wardrobe. All outfits must be age appropriate. Each model must also sell at least 2 tickets to this event before the day of the event.

Awards & Prizes
Scene Queens (5 Winners) will receive an award + gift bag

​Tee Party Top Model (1 Winner) will receive a crown, sash, award + gift bag

People’s Choice Award: The contestant with the most votes wins 25% of all People’s Choice proceeds! Votes will cost $1 each. This contest begins in April.

12:30 PM — Show Run-Through
1:00 PM - Hair & Make-Up/Vendors/Raffle
2:15 PM — Dress for Scenes
2:45 PM — Line-up
3:00 PM — SHOWTIME!

Christian Miss Organization – Event Participation Waiver & Release 
I understand that participation in Christian Miss Organization events, programs, workshops, and activities (“Events”) involves group activities that may include, but are not limited to, crafts, games, movement, food preparation, and general supervision in a group setting.

I voluntarily give permission for my child (or myself, if over 18) to participate in all Event activities.

 
Assumption of Risk
I acknowledge that participation in Events involves inherent risks, including but not limited to minor injuries, allergic reactions, or accidents. I knowingly and voluntarily assume all such risks on behalf of myself and/or my child.

 
Medical & Care Information
I agree to disclose any medical conditions, allergies, or additional support needs prior to the Event. I understand that Events are led by volunteers and staff who provide general supervision only and are not trained to provide specialized, medical, or one-on-one care.

 
Release of Liability
In consideration of participation, I hereby release and hold harmless Christian Miss Organization Inc., its directors, officers, employees, volunteers, partners, and affiliates from any and all claims, liabilities, demands, or causes of action arising out of or related to participation in the Event, except in cases of gross negligence.

 
Emergency Medical Treatment
In the event of an emergency, I authorize Christian Miss Organization representatives to seek medical treatment for myself and/or my child if I cannot be reached. I understand that I am responsible for all medical expenses incurred.

 
Photo & Media Release
I grant permission for Christian Miss Organization to photograph, video record, or otherwise capture my child’s or my likeness during Events for promotional, educational, or marketing purposes, including print and digital media, without compensation.

 
Behavior Expectations
I understand that participants are expected to follow instructions and behave respectfully. Christian Miss Organization reserves the right to remove any participant whose behavior poses a safety concern or disrupts the Event, without refund.
