I, the undersigned parent/guardian, acknowledge that participation in cheerleading activities involves inherent risks, including but not limited to the risk of injury. I voluntarily assume all such risks on behalf of my child. I understand that cheerleading may involve physical activity and could result in various injuries, including but not limited to fractures, sprains, and other serious injuries.
In the event of an emergency, I authorize Elev8 Athletics and its representatives to seek medical treatment for my child. I understand that every effort will be made to contact me as soon as possible
I, the undersigned parent/guardian, hereby release, absolve, and hold harmless Elev8 Athletics, its coaches, volunteers, and any affiliated organization from any and all liability for injuries or damages that may occur during my child's participation in the cheer tryouts or any related activities.
I have read this waiver and fully understand its contents. I am aware that this is a release of liability and a contract between myself and Elev8 Athletics, and I sign it of my own free will.
