This form is for reporting your NACUSA event. Events must have (1) this form completed and (2) a pdf of the program to upload. Chapter reports are due no later than the 1st of April (Jan - Mar concerts), 1st of July (Apr - Jun concerts), 1st of Oct (Jul - Sep concerts), and the 1st of Jan (Oct - Dec concerts). If you have a large number of events, please contact David Peoples for a fillable spreadsheet.
