The purpose of these Confidentiality Guidelines is to protect the privacy, dignity, and legal rights of students served by the Edge High School Success Coaching Program while ensuring compliance with applicable federal and state laws, including the Family Educational Rights and Privacy Act (FERPA) and Arizona mandatory reporting statutes.
All Success Coaches, mentors, and volunteers are required to comply with these guidelines as a condition of participation.

For purposes of this Agreement, Confidential Information includes, but is not limited to:

• Student education records and personally identifiable information
• Academic progress, attendance, behavior, or disciplinary information
• Personal, family, health, or social information disclosed by a student
• Information shared by Edge High School staff regarding a student
• Any written, verbal, electronic, or observed information obtained through coaching activities

Confidential information may be obtained through direct conversation, observation, documentation, electronic communication, or participation in meetings.

Success Coaches agree to:

• Treat all student information as private and confidential
• Share student information only with authorized Edge High School personnel who have a need to know
• Use student information solely for purposes related to supporting student success
• Maintain confidentiality both during and after participation in the Success Coaching Program

Confidential information may not be disclosed to friends, family members, other volunteers, or outside parties.

Success Coaches must understand that confidentiality is not absolute. Confidentiality must be broken when disclosure is required by law or school policy, including but not limited to:

• Suspected child abuse or neglect (mandatory reporting under A.R.S. § 13-3620)
• Threats of harm to self or others
• Court orders or lawful requests by authorized agencies

Success Coaches must never promise confidentiality to a student.

Success Coaches are mandatory reporters under Arizona law. Any reasonable suspicion of abuse or neglect must be reported immediately in accordance with Edge High School Mandatory Reporting Guidelines.

Reporting concerns in good faith is legally protected and required. Failure to report may result in removal from the Success Coaching Program and legal consequences.

Success Coaches shall:

• Avoid discussing students in public or non-private settings
• Use only approved communication channels when communicating about students
• Safeguard written notes, electronic files, emails, and messages containing student information
• Not retain, copy, photograph, or distribute student records or information

All notes or materials created in the course of coaching are considered property of Edge High School.

Success Coaches shall not:

• Postor reference students, student situations, or identifying details on social media
• Share photographs, names, or stories about students without written school authorization
• Communicate with students through social media

All electronic communication must comply with Edge High School policies.

Any breach of confidentiality may result in:

• Immediate removal from the Success Coaching Program
• Possible legal consequences

Suspected breaches must be reported immediately to an Edge High School administrator.

The obligation to maintain confidentiality continues indefinitely, including after a Success Coach's participation in the program ends.

I acknowledge that I have read and understand the Confidentiality Guidelines for the Edge High School Success Coaching Program. I agree to comply fully with these requirements and understand the limits of confidentiality, including my obligations as a mandatory reporter.

This Agreement shall be governed by and construed in accordance with the laws of the State of Arizona.
